Job overview

Derbyshire Community Health Service Trust (DCHS) has an exciting opportunity for a cardiac professional to join our cardiac rehabilitation team. 

The Cardiac Rehabilitation service covers North Derbyshire to deliver exercise and education to cardiac patients. 

We are looking for a Cardiac Nurse  / Physiotherapist with understanding and experience in the delivery of rehabilitation, and the confidence to integrate into a innovative and passionate team. 

A band 5 development position will be considered for the right candidate. 

Main duties of the job

The main duties of the role include, but are not limited to, assessing patients safety to enrol onto the exercise and education programme. 

Assisting in the delivery of safe and prescriptive exercise, whilst monitoring, recognising and then adapting exercise when required. 

You will be expected to travel across North Derbyshire, aiding in functional testing and exercise delivery. 

Contribution to service development and innovation will be key, as we continue evolve to meet the complex needs of our patients.

Detailed job description and main responsibilities

Support colleagues in the delivery of exercise, adapting where necessary and progressing when needed. 

Functional testing in clinical assessments. 

Telephone support and assessment of patients. 

Inputting patient data onto system one, AMaT, and at times the National Audit database.  

Contribute to innovative ideas for better working ways and better patient care. 

Administrative roles and activity, including digitisation.

Qualifications

Essential criteria
  • Registered nurse
  • Evidence of recent training relevant to cardiac rehabilitation
Desirable criteria
  • Evidence of experience in cardiology

Experience

Essential criteria
  • Experience of working with a wide range of cardiac conditions
  • Experience of working with long term conditions
Desirable criteria
  • Community experience

Skills

Essential criteria
  • Excellent communication and interpersonal skills
  • Good IT Skills /SystmOne experience/word processing
  • Ability to assess, plan and implement care
