Trosolwg o'r swydd

  • To work as part of the team to provide nursing assessments, interventions and nursing care to children, young people and families on the caseload.
  • To work as part of a mixed skill team in partnership with children and families/carers to implement individualised care plans to meet medical and nursing needs.
  • To carry a caseload and act as lead professional.
  • To provide leadership and act as team leader if required, advising schools and partners.
  • To support other bands of staff in gaining knowledge and skills to enable high quality care for children.
  • To participate as part of a skilled mix team and within sphere of competence.
  • To be an advocate for the child, young person and family to ensure their holistic needs are met.
  • Participate in an on call rota for children's community nursing teams.

Prif ddyletswyddau'r swydd

  • To make clinical judgements based on specialist assessment, planning, implementing and monitoring of the child in the community setting with a nursing need, categorised into four main groups (Department of Health, 2011)
  1. Children with acute and short-term conditions
  2. Children with long-term conditions
  3. Children with disabilities and complex conditions.
  4. Children with life-limiting and life-threatening conditions.
  • To work autonomously as well as part of a team/integrated agency approach in provision of high quality, evidenced based care
  • To support and supervise staff ensuring safe care in the community and minimise risk. This may include professional mentoring of junior staff and nursing students. Staff appraisals, 1:1’s and practice assessor role will be required.
  • To escalate situations to the appropriate professional or service, where there are a number of complicated aspects to take into account and a solution is difficult to identify. This may include advanced clinical assessment needing specialist medical review, requirement for specialist equipment to support a clinical need, additional support from a socio-economic perspective.
  • To analyse any concerns raised by the child/young person (CYP) and family or caregiver and resolve/signpost this accordingly. This may include an event, problem encountered with equipment, or recognition of signs and symptoms that require patient assessment and/or specialist management.

Due to  promotions within the service, we currently have two exciting opportunities to join the Dorchester's  Children's Community Nursing Team  as a Band 6 Children's Community Nurse.

This post holder will work within the Children's Community Nursing Team, visiting children in the community with a wide range of conditions and health needs.

The role will include nursing care, training and supporting families as well as deputising for senior team members and supporting student nurses.

Knowledge/Skills

Meini prawf hanfodol
  • Significant experience of children’s nursing post registration [RNC]
  • Knowledge and experience working with children who have Complex, Chronic, and palliative care needs
  • Experience of End of Life care and willingness to develop skills
  • Ability to recognise, take appropriate action and contribute to the child/adult protection process in order to safeguard children and young people
  • Ability to work in a flexible manner and prioritise according to the demands of the situation, time and workload management.
  • Understanding of Mental Capacity Act and Deprivation of Liberties as it applies to children and young people
Meini prawf dymunol
  • Understanding of professional nursing Issues and the pillars of clinical governance including experience of audit

Education & Qualification

  • Registered Children’s Nurse (RSCN/RCN)
  • Teaching Qualification or equivalent
  • Evidence of continuous professional development

  • Holds a degree in a relevant health related subject
