Are you a qualified Advanced Clinical Practitioner?

Come and join us at Lincolnshire Community Health Services NHS Trust

This is an exciting opportunity to work in one of our Community Hospital wards providing inpatient care within Lincolnshire. 

The ACP will use specialist knowledge and skills to provide healthcare autonomously to patients in their area. They will have accountability for independent clinical assessments, diagnosis, and treatment of patients with undiagnosed and undifferentiated conditions. In addition to this the ACP will also be responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.

Our care pathways include Palliative care and end of life; rehabilitation (this includes stroke patients stepping down from acute care) and transitional care.

We have 4 locations; Gainsborough, Louth, Spalding and Skegness.

Successful candidates will need to demonstrate that they are able to meet the essential criteria in the person specification and can provide evidence of significant experience in the treatment of long-term conditions, frailty, and palliation. Candidates will also hold a non-medical prescribing qualification.

We are looking for people who can demonstrate the right values and behaviours and have excellent leadership and communications skills.

As part of a team, you will work as an autonomous practitioner, with support from our ward medical practitioners.

Detailed job description and main responsibilities

You will be required to have and an MSC in Advanced Clinical Practice.  Candidates will also hold a non-medical prescribing qualification.

There may also be an opportunity to work with our Urgent Care colleagues to gain insight and experience in this area of care.

The current vacancy is for a full-time role working day shifts pro rata over a 7-day period.

Qualifications

Essential criteria
  • Registered Health Care Professional holding current registration with appropriate regulatory body.
  • Postgraduate study in Advanced Clinical Practice/Autonomous Practice at Masters level
  • Non-Medical Prescribing qualification
  • Safeguarding-level 3 or willingness to undertake
  • Teaching and assessing qualification

Previous Experinece

Essential criteria
  • Evidence of recent and on-going personal development including obvious leadership development
  • Experience of education and training issues/requirements for undergraduate and post graduate members of professions.
  • Extensive post registration experience in relevant settings, including experience in the management of long term conditions, frailty and palliation.
  • Demonstrates specialist knowledge of operational management, education and leadership
  • Skills for analysis of service delivery and improvement planning.
Desirable criteria
  • Able to demonstrate a working knowledge and experience of the governance agenda locally.
