Job overview

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

We are looking for clinical professionals from a range of clinical backgrounds who are currently working in a community-based setting within Bexley, who can help design, improve, and better join up service provision for those in the borough living with multiple long-term conditions. You will be driving forward change, working across organisational boundaries with the lived experience of patients being at the heart of implementing service improvements. The key focus of all programmes of work is to reduce health inequalities and improve population health outcomes, with an emphasis on care being delivered close to home wherever possible and supporting patients to self-manage and live well with their condition.

More than one in four of the adult population in England lives with two or more conditions. Living with numerous and often complex health problems is becoming the norm, particularly for those living in disadvantaged communities. Some conditions cluster together and people can experience many different combinations of conditions. 

Main duties of the job

These positions offer an exciting opportunity to deliver sustainable improvements in the physical and mental health outcomes for the growing number of people living in Bexley who have one or more long-term conditions such as diabetes, CKD, cardiovascular disease, hypertension and respiratory disease. A multi-morbidity approach becomes increasingly key and the development of joined up multi-disciplinary models of care that span our Local Care Networks, will be the model that evolves and develops over the next three to five years.

Strategically, the post-holder will represent the needs of Bexley residents at forums within the borough and at a SEL level through participation in collaborative networks that enable the sharing of best practice. Operationally, the role will be the go-to point for all thematic queries and concerns relating to long term condition pathways locally.

Detailed job description and main responsibilities

We are therefore seeking one or two clinical professional(s) who are currently working in a front-line, patient facing role in the Bexley Healthcare System and is committed to taking on this leadership position 1-3 sessions per week on a fixed term contract of employment or secondment.  This individual needs to be passionate about improving patient care through working collaboratively with other service provider partners, to make the strategic ambitions of more integrated care an operational reality in Bexley.

Qualifications

Essential criteria
  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area
Desirable criteria
  • Certificate / qualification in programme management, service improvement or transformation

Experience

Essential criteria
  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
Desirable criteria
  • Previous experience of being part of peer networks and collaborative working
  • Cross organisational and/or professional boundary collaboration experience

Knowledge

Essential criteria
  • An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
  • Champions the principles of equality, diversity and inclusion both within and outside of the organisation. Provide effective, inclusive and compassionate leadership ensuring all voices are heard and creating and nurturing a culture where staff feel safe to speak up.
Desirable criteria
  • Good knowledge or cross professional transformation, digital solutions and innovative redesign.
