Bromley Older People and Frailty Clinical Lead

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The primary responsibility of the Clinical Lead is to provide clinical input and support to the One Bromley Local Care Partnership (LCP) in its objective to improve the quality and delivery of frailty, EOL and care home services in Bromley.
The Frailty element of the role will cover proactive care, joint work on urgent community response (with the UEC lead) and acute frailty.
The Clinical Lead will act as a clinical champion and support members of the LCP in developing clinically effective and efficient pathways that provide high quality care to patients in the most appropriate setting to meet their clinical lead. For the care homes work, the postholder will link closely with the Bromleag Care Practice, UEC and EOL workstreams. These areas of work are led by experienced senior managers with whom the postholder will work jointly.

• Provide strategic leadership into the programmes of work supporting these areas of work, aligned to the One Bromley strategic plan, in partnership with One Bromley Providers
• To provide clinical input to develop and deliver the developing strategic integrated frailty model, leading the One Bromley Frailty Task & Finish Group and other internal subgroups that have or may need to be established to develop and facilitate the development of frailty pathways
• Working closely with Care Homes themselves, the Care Home practice (Bromleag), Learning Disabilities teams and Mental Health Care homes, the role will support the delivery of integrated, equitable care for care home residents.
• End of Life care is a key topic in Bromley, and the postholder will lead the One Bromley system to deliver a high quality, co-ordinated end of life care offer for Bromley patients.
• A current focus for Bromley is increasing adoption Universal Care Plans, using the opportunity to further promote and embed the use of shared care records and improve quality of Advance Care Planning.
• Leadership on access to EOL medication out of hours, embedding an effective communication tool to co-ordinate the care of rapidly dying patients at home and looking at how we can build capacity across primary care and social care to support palliative patients as well as improving the transfers of care between services are important current elements to the role.

Qualifications

- Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.

Experience

- Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.

- An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
