Job overview
An exciting opportunity has arisen for a highly motivated, Health Support Worker to join our team. The successful candidate will have the opportunity to work as an integral member of a multidisciplinary team which provides expert, caring and holistic support to adults with a diagnosed learning disability.
You will be supported to develop skills to carry out a variety of interventions delegated by nursing or allied health professionals (Speech and Language, Occupational Therapy, Dietetics and Physiotherapy). You will work with people who have a learning disability and physical health care needs such as issues relating to communication, mobility, nutrition and dysphagia. You will be required to work in a variety of community settings, with individuals, groups, families and staff teams.
You will have regular supervision and will be expected to support the multidisciplinary team as a whole. Duties may include direct intervention but also developing resources and supporting training.

Main duties of the job
- To support nursing and allied health professionals (Speech and Language, Occupational Therapy, Dietetics and Physiotherapy) within the multi-disciplinary team to deliver safe and person-centred care.
- Work with service users to provide interventions through ongoing support, practical assistance and problem resolution under the supervision of a senior clinician.
- To contribute to the regular assessment, and reviews of services users’ needs, development of care plans and provision of appropriate interventions.
- To work in partnership with other agencies, teams, and carers to meet the needs of services users.

Detailed job description and main responsibilities
- To support service users with a range of needs under the supervision of a clinician.
- Under the direction/supervision of clinicians to assist with screening of referrals and client assessment.
- Work with service users to facilitate access to primary and secondary healthcare services.
- Assist members of the MDT to prepare deliver training and group sessions.
- Following assessment/review by clinicians assist to monitor services users holistic health and report any changes to the appropriate clinician.
- Undertake tasks delegated by supervisor and team manager.
- Support the multi-disciplinary team to deliver safe person-centred care.
- Develop resources for the team and services users; including accessible information.
- Create and maintain data to support effective running of the team.
- Ensure that the individual’s rights and choices are actively promoted and supported to ensure they ae appropriately acknowledged.
- Provide support and assistance to professionals within the team.
- Liaise with health & social care agencies and care homes to meet services users’ needs.
- Visiting service users in a variety of settings including their home, work and day placements, hospital settings and group home, after carrying out an appropriate risk assessment.
- Maintain accurate records in accordance with policy and procedure.
- To have an understanding of the risk factors and health & safety issues involved in the provision of care to services users, families and self.
- To adhere to protocols, policies; including local Lone working policy.
- Assist to develop and maintain robust and up to date care plan and risk summaries.
- To develop a rapport with service users through professional support and collaboration within appropriate boundaries.
- To assist service users to maintain activities of daily living.
- To raise any concerns regarding service user or care provision promptly.
Person specification
Experience
Essential criteria
- Experience of working with people who have a learning disability.
- Experience of working in a community setting
- Experience of undertaking assessment of the needs of people with learning disabilities and developing a robust care plan to meet needs.
Desirable criteria
- Experience of teaching and training of staff and/or carers
- Experience of contributing to policy and procedure development and implementation.
- Experience of managing and supervising other staff
- Experience of developing and delivering training packages with other professionals
Education and Qualification
Essential criteria
- RNLD (current NMC registration)
- Evidence of continuing professional and personal development
Desirable criteria
- Mentorship training
- Facilitator’s training
- ENB 998 or equivalent
- Post registration training in Learning Disabilities
Knowledge & Skills
Essential criteria
- Knowledge of the predominant health needs of people with learning disabilities
- Knowledge of assessment tools for people with learning disabilities
- Knowledge of the health inequalities experienced by people with learning disabilities and how these can be addressed
- Knowledge of our business and how it supports patient care
- Good organisational ability and time management skills.
- Effective communication skills both written and verbal.
- Good IT Skills
- Skills in assessment, care planning and case management.
- Knowledge of relevant legislation and how they apply to people with learning disabilities.

- Research and audit skills.
