We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

 

Job overview

West London Trust (WLT) is a dynamic healthcare organization committed to delivering high-quality care to our community. We are currently seeking a dedicated and experienced Service Manager to join our Integrated Care team based at Clayponds Hospital.

As the Service Manager for Access and Admission Avoidance Pathway, you will play a pivotal role in overseeing and coordinating community-based admission avoidance pathways. This includes managing rapid response and home first services, as well as access pathways such as discharge and referral hubs.

Main duties of the job

As the Service Manager for Community-Based Admission Avoidance Pathways and Access Pathways, you will play a crucial role in facilitating seamless transitions of care for patients. Your responsibilities will include providing strategic and operational leadership for admission avoidance pathways, discharge hubs, referral hubs, rapid response, and home first services. This involves managing the delivery of rapid response and home first services, ensuring timely and effective interventions to prevent unnecessary hospital admissions.

 

Collaboration with multidisciplinary teams will be essential to develop and implement innovative strategies aimed at improving access to care and reducing hospital admissions. You will lead service development initiatives, working closely with stakeholders to identify and address any gaps in service provision.

 

Compliance with regulatory standards and best practices in admission avoidance pathways will be a priority, and you will be responsible for monitoring service performance and outcomes. Implementing quality improvement measures as necessary will also be part of your role.

 

Providing leadership and support to staff will be integral, fostering a culture of continuous learning and development within the team to ensure high-quality care delivery.

Qualifications & Training

Essential criteria
  • Relevant professional qualification (Professional registration holder)
  • Appropriate professional registration e.g. NMC, HCPC
  • Educated to degree/ diploma level with a relevant qualification or equivalent experience.
  • Clinical/post graduate management qualification to masters or equivalent level of experience and proven ability to perform at this level

Experience

  • Relevant operational experience to manage unplanned/admission avoidance and access pathways.
  • Extensive experience of and demonstrable achievement in managing health/social care services.
  • Management of resources and good track record with change management, efficiency savings and financial recovery plans.
  • Experience of developing and maintaining effective partnerships with stakeholders in the redesign of services.
  • Experience of managing a diverse practitioner team (multi-site).
  • Measurable track record in involving and motivating staff to improve performance.
  • Knowledge of team development and workforce requirements for modern health and social care services.
  • Understanding of clinical governance framework and its application in practice
