Newly developed Hospital Discharge Team & Community Home First Hub Service.

An exciting opportunity is now available for the correct Social Work candidates to join a hybrid social care services supporting patient in the acute setting and those patients placed to Discharge to Assess & Long-Term facilities.

This role is based over two facilities, one acute and one community which both support multidisciplinary working. As part of this multidisciplinary approach, you will work hand in hand with therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay.

Here at the Northern Care Alliance the social care teams work 37 hours weekly which covers 7 day working via a team rota which include days/ evenings/weekends.

Main duties of the job

As a Social Worker in the Hospital Discharge Team & Community Home First Hub Service you would have the following responsibilities;

  • Be person centred and reinforce a patient’s choice and control.
  • Promote a patient’s well being throughout their journey in the acute and community setting.
  • Provide Strength Based Assessments and independence.
  • Complete Care Act 2014 compliant assessment.
  • Source provisions both formal and informal in support of patient needs.
  • Interact with a plethora of professionals daily.
  • Guide and support colleagues from other professions or colleagues less experienced in social care processes.
  • Have knowledge of related legislation, policy, and frameworks.
  • Experience of Safeguarding processes and procedures.
  • Complete mental capacity assessment and best interest processes for patients unable to make specific decision.
  • Knowledge of the Deprivation of Liberty Safeguards.
  • Be an active and integral part to the team.

From commencement you will be supported by a dedicated teams from Practice Development, Leadership and Learning Development. This support includes access to specific programmes, e-learning, training courses, shadowing opportunities, partnership led workshops and bespoke CPD funding to help you access university modules linked to your goals and objectives to take your career in the right direction.

Qualifications

Essential criteria
  • Possession of recognised Social Work Qualification: SW Degree, CQSW, CSS, Dip SW or equivalent
  • Social Work England Registration

Experience

Essential criteria
  • Clearly evidence (in your practice) an understanding of the principles and values underlying the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation and guidance such as Continuing Healthcare etc
  • Clearly evidence your ability to develop effective working realtionships with service users and their carers (in your practice) and show evidence of good assessment practice.
  • You must clearly evidence that you have the ability to maintain accurate records to communicate complex information clearly and concisely both verbally and in written form
  • You must clearly evidence that you have a clear understanding of the social work role in a multi-disciplinary setting.
  • Demonstrate the ability to develop effective working relationships with other professionals and colleagues.
  • Possess skills of negotiation, mediation and de-escalation - seeking constructive approaches to problem solving.
  • Experience of working with adults/older people.
  • Have a good standard of computer skills.
Desirable criteria
  • You have additional qualifications; Best Interest Assessors; AMHP; Practice Educator; Post Qualifying courses or other professional development qualifications.
  • You can evidence an innovative approach to the development of new methods of working to support people in the community.

Knowledge

Essential criteria
  • You must clearly evidence your understanding of the integration agenda and the promotion of independence (enablement/strength based practice) model of health and social care
  • Demonstrate a thorough understanding of personalisation, person centred and strength based practice
Desirable criteria
  • You can show knowledge of relevant Welfare Rights Legislation.
  • You can show knowledge of the recent Government Guidelines.
