Job overview

Are you ready for a challenge? The Discharge Team is seeking to appoint Discharge Coordinators. The role of the Discharge Coordinator will be to support the Discharge to Assess process, facilitating safe discharges from the hospital and aiding flow and capacity within the Trust. This position entails a seven-day service, including bank holidays. Current shift patterns will be a variety of 8:00 AM to 4:00 PM, 9.00AM to 5.00PM and 10:00 AM to 6:00 PM. The role involves working across all clinical areas and may require duty at other Sherwood Forest sites (Mansfield Community Hospital and Newark Hospital) as needed.

Candidates should possess a clear understanding of the Discharge to Assess process, sound knowledge of discharge pathways, and awareness of social care and health processes. Full training will be provided for this role

Knowledge

Desirable criteria
  • Experience in managing patient flow.
  • Community resource experience
  • NHS social care or similar background
  • Care experience
  • Previous experience of admission/discharge processes
  • Computer skills
  • Knowledge of Trust policies

Qualifications

Essential criteria
  • NVQ level 2 or equivalent
  • Good standard of general education including GCSE English or equivalent.
  • Audit Knowledge
Desirable criteria
  • ECDL
  • NVQ in Promoting independence or equivalent in Care
  • NVQ in Health and Social Care

Further Training

Essential criteria
  • Willing to undertake further training for personal development and for knowledge and skills development.
  • Mandatory training
Desirable criteria
  • Understanding of the Delayed transfer of care act
  • Understanding of the National framework for NHS continuing Health Care
  • Knowledge of Medway

Experience

Essential criteria
  • Ability to work as a member of a team, to cope with stress, to be self-motivated and to be structured regarding work schedules.
