This is a senior Nursing/Midwifery role within the organisation and there is an expectation that the post holder will fully participate in the leadership and delivery of high quality patient care within the Care Group.

The post holder will be a practitioner within his/her specialty field of Diabetes and Endocrine conditions.

The prime function of the role is to provide clinical expertise in supporting the delivery of high standards of nursing care for patients within his/her care.

Work collaboratively with the multi professional team to develop and maintain a quality service for patients within specialty

To provide expert clinical nursing, education and advice in all aspects of the specialty.

Facilitate an improvement in the quality of health and outcome of care to this client group.

To act as a source of expertise and knowledge for patients, carers and health professionals as well as other agencies involved in this specialty.

To work with multi-disciplinary teams both in hospital and the community, in the provision of optimal care, to improve quality of life.

The post-holder is responsible for supporting the continuous improvement of quality of services and safeguarding high standards of care by sustaining an environment in which excellence in clinical care can flourish.

  • Promote clinical excellence through his/her practice in the specialty.
  •  Make autonomous decisions as required. 
  • Work collaboratively in the Multidisciplinary team.
  • Provide specialist advice and support to patients and carers.
  • Responsible for own caseload
  • Enable and empower patients to make informed decisions regarding treatment options and non- medical prescribing. 
  • Responsible for maintaining accurate, up to date clinical records and other relevant documentation using manual /IT systems. 
  • Contribute to the development of clinical guidelines and protocols. 
  • To use relevant specialist training, expertise and knowledge within the specialty with treatment options.  
  • Undertake surveys, audit and research and development, through evidence-based practice, demonstrate clinical excellence with measurable standards and outcomes aiming to improve care and quality of life.
  • Work in accordance with the NMC. code of professional conduct. 
  • The post holder will have a portfolio of relevant up to date evidence of clinical practice within his/her specialist field.
  • Use clinical supervision and peer group support to underpin personal professional development.
  • Take responsibility for driving personal annual appraisal process and annual job planning process.

Experience
