Would you like the opportunity to work in a specialist Veteran mental health team?

We are looking for an experienced mental health practitioner / community nurse to join our lively and passionate mental health team; supporting veterans and reservists presenting with mental health problems and their families. Some of our team have a personal connection to veteran's, either with a military background or holding a close relationship with someone who does.

This role of Family Case Worker is looking to increase the services' family and carer support. It will include direct contact with families for assessment, intervention and engagement with signposting to other agencies.

We cover Kent, Surrey and Sussex and deliver assessments, support and psychological therapy within our Op Courage team. This post will be based in Kent and cover all regions through a mix of face-to-face and remote appointments.

Main duties of the job

The post will be working alongside colleagues within an MDT and external stakeholders and veteran charities.

Working collaboratively as part of the Op Courage service, the postholder will identify need and create support pathways for families of veterans receiving support from Op Courage.

The post holder will manage a case load of families who require additional support to stabilise the family system and offer the best opportunity for the veteran and the family to recover. The role will include assessment, recovery planning and co-ordination for families, as well as provision of welfare support, guidance and advice. In liaison with other Op Courage colleagues, the postholder will be responsible for enhancing the veterans’ family’s sense of security, self-management, independence, and their ability to improve overall quality of life.

This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).

We are an expanding and exciting specialist service within SPFT, working regional in the south east and nationally as part of the NHS armed forces networks for Op courage.

Detailed job description and main responsibilities

To participate within the multi-disciplinary professional team in the relevant care group to deliver safe patient care focussing on therapeutic relationships.

Take a lead role in identifying Op Courage Veterans’ families with additional needs including mental health, financial and support needs.

Develop brief interventions to support family members in their support of Veterans.

Have an understanding of nursing interventions relevant to the care group to include education, clinical care, wellness recovery action plans and discharge planning.

Essential criteria
  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role.

Knowledge/Experience

Essential criteria
  • Knowledge of MHA (1983/2007)
  • Knowledge of Mental Capacity Act (2005), to include practical knowledge of FACE documentation, DOLS and Best Interest decisions
  • Child and Adult safe guarding procedures.
  • Change management at organising and prioritising their workload
  • Demonstrate ability to enhance care through evidence based knowledge
  • Demonstrate professional development relevant to the specific care group
