Job overview

An exciting opportunity has arisen for an Advanced Care Practitioner  to join our Stroke team.  We are looking for a highly motivated, knowledgeable and educated candidate to join our team. The post is also open to trainee ACP's 

The successful candidate will work autonomously making high level decisions around the care and treatment of patients within our Stroke service.

The inpatient Stroke unit consists of 48 beds - based at the Calderdale Royal Hospital. 

 

 
 

Main duties of the job

Role is an integral part of the Stroke Team. Involves working with the Stroke Physicians - including working in TIA Clinic. Providing cover for outpatient for clinics.  Carrying out ward rounds and leading patient and family meetings, and Community care follow up clinics.

Advanced level practice encompasses aspects of education, research and management but is firmly grounded in direct care provision or clinical work with patients’ families and populations.

Detailed job description and main responsibilities

assess individuals, families and carers, holistically using a range of methods, some of which may not be usually exercised by nurses such as physical assessment and history taking, ordering and interpreting diagnostic tests or advanced health needs assessments • Communicate effectively with patients carers and the multidisciplinary team. • Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnoses. • Decide on and carry out treatment, which may include the prescribing of medicines, or referring patients to an appropriate specialist. • Ensure the provision of continuity of care. • Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed. • Work autonomously within a wider health care team and as a lone practitioner. • Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.

Qualifications

Essential criteria
  • Registered Nurse or Registered Allied Health Professional
  • Masters in Advanced Practice or equivalent
  • Independent Non- Medical Prescriber
Desirable criteria
  • Teaching/Assessment, Mentoring qualification

Experience

Essential criteria
  • Currently working as an autonomous practitioner in advanced practice.
  • Working knowledge and clinical management of patients specific to role

Training

Essential criteria
  • Previous experience of formal/informal teaching of patients and staff

Special Knowledge/Expertise

Essential criteria
  • Demonstrates up to date knowledge and expertise of Advanced Practice.
  • Knowledge of the key professional issues and professional regulation guidelines related to professional practice
  • Evidence of being able to work across organisational boundaries
  • Experience of applying research evidence to clinical practice.
  • Evidence of continuing professional development
  • Commitment to maintain own fitness for practice through maintaining a personal profile of competencies (profile)
