Job overview

We are recruiting to a brand new, exciting post and are looking for an experienced social worker to join the expansion of an existing service and help develop the integration of social work into the overall service. The Forensic Outreach Liaison Service (FOLS) provides specialist community support and intervention for service users with ongoing significant/complex mental health needs who present a significant risk to others related to their mental disorder. The service will be based at Clifton House in York, providing a service to York, Selby and other parts of North Yorkshire. 

We’re on the lookout for a motivated and dynamic social worker to be an integral part of FOLS. In addition to care co-ordinating a small community caseload, the team will provide in-reach work to the inpatient wards at Clifton House, utilising their expertise to assist discharge from secure services and ensure that the transition into the community is seamless and timely. The FOLS will also work with established community services and stakeholders to provide risk management advice through a tiered consultation model, provide specialist interventions and assessments and offer training to agencies to support people to remain in their local areas.

Main duties of the job

•    Provide a Safeguarding lead within Forensic Services

•    To be a member of the leadership team within Forensic Services

•    To work as a senior team member within the Community Forensic Outreach team supporting patient pathways across the region 

•    To provide a lead role for transition and discharge for service users within Forensic Services, working closely with partner providers within the region 

•    To work as a Care Co-ordinator in partnership with service users and other Professionals’ with-in the Care Programme Approach framework managing and co-ordinating complex packages of care

•    To act as an autonomous senior clinician being accountable for a defined caseload of service users with complex needs 

•    To manage self and lead others in accordance with the Health and Care Professions Council (HCPC) recognizing limitations, demonstrating ability to practice within ethical and legal frameworks with clear understanding of service users, and their families' civil rights

•    To support, supervise and contribute to the professional development of members of staff working in Forensic Services

Detailed job description and main responsibilities

Whilst they will work as a member of a highly skilled multi-disciplinary team (MDT), delivering excellent quality care to our service users and carers, there will also be an expectation that the person will sometimes work autonomously, case managing a small number of patients detained in other hospitals. Applicants should be able to prioritise conflicting needs under pressure and will have experience of identifying and implementing care pathways, in liaison with other partnership agencies. The post holder will be the lead professional for Safeguarding within the service. Previous experience of supervising junior members of staff is also desirable as this post carries some leadership responsibilities within the service.

The ideal candidate will be enthusiastic, emotionally resilient, and able to work with individuals who challenge services.  The successful candidate must have current HCPC registration and will be expected to be able to travel to sites across a wide geographical area, so being a driver would be advantageous.

•    To participate in the mentorship, education and supervision of students from a range of disciplines 

•    To engage statutory and voluntary services to support the smooth transition of patient’s throughout the Forensic Pathway

•    To engage with Service Users accessing in-patient services outside of the Region and to support repatriation to services nearer their home

•    To provide a senior role in multi-disciplinary and multi-agency forums
•    Provides advice and guidance to other staff on complex cases
•    Form part of the senior leadership team contributing to the on-going development of Forensic Services to maintain comprehensive and up to date written records of assessments, CPA plans, evaluations and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.
•    To ensure effective communication and collaboration with others through attendance at, and chairing of CPA meetings, team meetings, ward rounds, clinical meetings and meeting held by partner agencies.
•    To work in partnership with Service Users and partner agencies (e.g. housing, Benefits, Social Services etc.) to facilitate benefit claims and other financial and housing support.
•    To complete complex social and statutory reports.
•    To ensure the service user and their families are central to and fully involved in the plan of care and have copies of care plans.
•    Imparting sensitive and complex information to service users and their families.
•    To be aware of and adhere to all professional and Trust Policies including the reporting of incidents and accidents, complaints procedures and the management of confidential information.
•    To maintain links with carers and ensure that Nearest Relatives are informed of their rights under the Mental Health Act, 1983 
•    Support Carers to access Carers Assessments, supporting carers to make contact with Local Authorities. 
•    Ensure that Statutory Social Circumstances reports are available for mental health tribunals and/or hospital manager’s hearing.
•    Ensure that the needs of service users children are understood, making referrals to social care where required and ensuring that all relevant parties are involved in child visiting arrangements.

Essential criteria
  • Registered Social Worker Part 3 or Part 13 of NMC registry.

Experience

Essential criteria
  • Minimum of 2 years post registration experience
  • Leadership experience and experience of autonomous practice in a complex setting
  • Previous experience of working within a community and/or forensic setting.
  • Experience of risk assessment and risk management.
  • Experience of training others specifically related to risk management or safeguarding.
  • Experience of working as part of a team and providing leadership within a team

Skills
