The Bracknell Community Mental Health Team for Adults has an opportunity for a clinician passionate about supporting adults with mental illness and those individuals working with them. We are looking for a new Clinical Team Lead to join our dynamic multi-disciplinary team operating Monday – Friday during the hours 9:00am – 5:00pm.

The post holder will manage our Task Team, providing vital task focused short term interventions for patients within the team, and lead on the new One Team assessment function of the service. The latter will involve both undertaking assessments and supporting the MDT discussions and decision making. This means the post will present an opportunity to provide essential clinical leadership for the service, ensuring our resource is used appropriately to enhance the experience of the patients on our caseload. Your clinical expertise will also be put to use in managing a small but complex caseload of clients.

The Bracknell team is an integrated service with social care and we are committed to this model in supporting our patients holistically. For the post holder this will mean working alongside, being supported by and supporting social care colleagues in the valuable work we do.

• The post holder will be responsible for providing continued clinical advice, training and supervision to a group of staff operating within a multi-disciplinary team.
• The post holder will direct and support the team’s allocation of new patients to staff involving a robust understanding and formulation of their risks, presenting concerns and the recommendation for ongoing care including, sometimes, transfer to the most appropriate care pathway. 
• The post holder will be responsible for the supervision of those staff tasked with providing care to patients presenting with significant complexity and risk and ensure that agreed policies are followed and local arrangements are in place to minimise risk.   
• The post holder will be responsible for the synergistic working with partner organisations including the relevant Local Authority.
