Job overview

This is a locum post for six months in the first instance.  The post-holder will contribute to the Interstitial Lung Disease (ILD) Service, based at The Hammersmith Hospital (HH) at Imperial College Healthcare NHS Trust. 

We are a tertiary, NHSE commissioned prescribing service, with dedicated ILD Coordinator, ILD Clinical Nurse Specialist, and ILD Pharmacist support. The Hammersmith currently runs 7 consultant-led ILD clinics a week, including a sarcoidosis clinic; a pulmonary vasculitis clinic alongside renal, rheumatology, and ENT; and a CTD-ILD clinic alongside rheumatology. There is a quarterly scleroderma ILD clinic. There are weekly ILD radiology MDT and clinical MDT meetings, a weekly vasculitis MDT, and a monthly cardiac sarcoid MDT. There are day case admission facilities at Hammersmith and Charing Cross sites. There is close collaboration with the Hammersmith National Pulmonary Hypertension Service, and the Harefield and Papworth lung transplant services. 

Main duties of the job

The post-holder will also contribute to the acute medical take at Charing Cross Hospital (CXH) and participate in the respiratory ward consultant rota, which includes the care of patients with acute primary respiratory failure requiring non-invasive ventilation on the recently-refurbished Acute Respiratory Unit at CXH. 

This post, therefore, represents an opportunity to deliver excellent care to patients with respiratory disease, including ILD, in a specialist tertiary NHS England commissioned centre, supported by a well-established multi-disciplinary team.  It would be suitable for both a post-CCT candidate looking to develop as a consultant in a supportive team or for a more established consultant looking to move to a larger organisation.

Detailed job description and main responsibilities

On appointment, the candidate will have a formal job plan meeting with the clinical director when the candidate will have the opportunity to make changes to the time-table to suit the candidate’s needs.  

During acute medicine, all other clinical commitments are cancelled. Respiratory ward commitments never overlap with acute medicine commitments. 

In ILD clinics new patients are allocated 30 minutes per slot and follow ups 15 minutes per slot.  A typical clinic includes 5 new cases and 2 follow ups, but this will vary with demand on the service.  These slots may be used flexibly for new or follow up patients as needed.

Qualifications

Essential criteria
  • GMC registered Medical Practitioner
  • MRCP(UK) or an equivalent qualification
  • Entry on the General Medical Council (GMC) Specialist Register via one of the following: a. Certificate of Completion of Training (CCT) (the proposed CCT date must be within 6 months of the interview) b. Certificate of Eligibility for Specialist Registration (CESR) c. European Community Rights

Knowledge/ Skills

Essential criteria
  • CCT in Respiratory and general internal medicine
  • Independent practice in bronchoscopy
  • Pleural ultrasound level 1
  • Experience of working in an ILD tertiary service
  • Experience of working in ILD clinics
  • Competent in acute use of NIV, CPAP and Highflow Nasal oxygen
  • Experience of leading acute unselected medical take
  • IT Skills and computer literacy

Leadership/ Management Skills

Essential criteria
  • Able to take responsibility, show leadership, make decisions, exert appropriate authority.
  • Have proven skills in leading, motivating, developing and managing the performance of colleagues.
  • Knowledge of finance/budgets
  • Management of staff

Teaching & Training

Essential criteria
  • Experience of teaching and training undergraduates / postgraduates and junior medical staff

Audit

Essential criteria
  • Understanding of principles of clinical audit
  • Leading audits
  • Experience of submitting and reviewing data for national lung cancer clinical audit

Academic achievement

Essential criteria
  • Evidence of achievement appropriate to appointment at consultant level at ICHT

Language

Essential criteria
  • Able to speak and write English to the appropriate standard necessary to fulfill the job requirements
