An exciting opportunity has arisen within the Care Homes Service in Leeds; we require a Band 5 Registered Mental Health Nurse to join the current multidisciplinary team in the Intensive Care Homes Treatment Team who work to support individual service users residing in care homes and provide a robust alternative to hospital admission by preventing the breakdown of a caring situation in a residential or nursing home.
The service users may present with a wide variety of conditions including dementia, depression, anxiety and psychosis, with a gradient of severity and identified need. We have a clear focus on non-pharmacological approaches and person-centred care.
You will have post registration experience and be able to demonstrate an in depth understanding of the needs of Service Users with dementia and complex mental health needs. You will be a car driver with a full UK driving licence, to enable you to travel independently for assessments and reviews.
We shall provide you with specialist training and development to ensure you are fully supported in this role.
Broadly, the model of service delivery includes: timely assessment of mental health presentation and risk, role modelling appropriate behaviours / techniques, education for care staff tailored to the needs of the individual service user, fast access to mental health, psychological approach and pharmacological advice.

We aim to help older people in their transition from hospital to care home and also prevent unnecessary hospital admissions by supporting care homes. You will be part of an MDT consisting of enthusiastic occupational therapists and nurses, with medical support from sector Consultant Psychiatrists and their colleagues. This is a challenging and rewarding role requiring liaison with care home staff, social workers, inpatient wards at The Mount (Older Peoples Mental Health Inpatient Unit) and LTHT (Leeds General Hospitals), as well as direct contact and assessment of older people in care homes and liaison with their families.
You will have the desire to enhance the wellbeing of others & promote recovery. You will work with your team to provide high quality person-centred care to our service users and provide written recommendations and guidance for care home staff on approaches to care, including occupation and meaningful activity, aspects of personal care, therapeutic observations/interventions & maintaining safety/minimising risk.

- Communication/Relationships - The post holder is expected to demonstrate articulate and effective written and verbal communication skills, with service users and their carer’s and a wide range of other people/professionals.
- I.T. - The post holder must maintain and produce detailed contemporaneous and accurate records and reports, in accordance with agreed procedures. IT literacy skills and the ability to work with electronic care records.
- Knowledge, skills, experience – Recognised professional qualification - Degree or Diploma Mental Health Nursing.
Will maintain current specific professional and legal standards of practice in accordance with professional codes of conduct e.g. NMC.
To hold an understanding and knowledge of the problems and difficulties experienced by older people with mental health illness. To have an understanding of the MHA, MCA and DOLS. Knowledge / experience of partnership working with other services. The post holder will engage in the learning of student nurses, professionals, carers and service users and support/supervise junior staff.
- Development – The post holder will participate in service development and ensure that own continuing professional development is undertaken.
Planning and Organisational Skills - Ability to assess and plan to meet complex needs and manage complex situations.

The post holder must be able to act as a role model for others, and demonstrate a high level of clinical judgment, decision-making and communication skills.
You will be part of an MDT consisting of occupational therapists & nurses, with medical support from Sector Consultant Psychiatrists and their colleagues. The service is commissioned to operate over 7 days, 08-20.30 hours weekdays and 10-18.00 hours weekends and bank holidays.
The team manages a varied and complex shared caseload which will include completing holistic and specialised assessments and therapeutic interventions, and providing detailed risk management plans. You will liaise with colleagues and attend multidisciplinary meetings.
Service User and carer involvement is at the centre of our approach, and this should be reflected in your values and practice.
You will act as shift coordinator on a rostered basis, triaging referrals into the team, providing information, support and signposting as required.
The right candidate will be supported into the team with an induction and then in developing and furthering their professional skills through reflection in appraisal objectives, management and clinical supervision and further training and development opportunities.
