Tŷ Hafan is a beautiful children's hospice located on the picturesque Vale of Glamorgan coastline. Covering South, East and West of Wales Tŷ Hafan offers a beautiful, newly refurbished working environment set in idyllic ground a short distance from the metropolitan area of Cardiff. 

We have an exciting opportunity to join our established Family Wellbeing and Outreach Services team as a Transition Support Worker within the hospice and in the community.

About the Role
This role is focused on delivering practical, emotional, and social support to young people with life-limiting conditions as they transition into adulthood, generally between the ages of 14 and 25, as well as providing support to their families. Under the guidance of the Transition Nurse, you will manage a caseload of young individuals, offering personalised support that aligns with each person's care plan. Much of the role will involve lone-working, visiting family homes and engaging with young people in community settings. You will also have responsibilities for organising and leading social events and supporting the Transition Nurse in the set-up of transition clinics.

Additionally, this role involves assisting the Transition Nurse and other professionals in delivering a broad spectrum of psychosocial services to enhance the wellbeing of young people and their families. You will also work closely with the Family Support team to help develop bereavement support services that address the specific needs of each child and family.

Please note that this role requires some flexibility and on occasion, may be required to work a weekend or evening. We are happy to discuss this and would provide as much notice as the circumstances would allow. Hours can be worked across the week upon discussion. 

About you
We are looking for someone who is approachable, empathetic, and capable of working both independently and as part of a team in a setting that can be emotionally challenging. The ideal candidate will have a background in community or multidisciplinary roles, particularly in support of young people and families with diverse and complex needs. You should be committed to a person-centered approach, ensuring that the preferences and voices of young people guide the support they receive as they prepare for adulthood. The ability to keep clear and accurate records is essential, as well as the ability to communicate effectively with colleagues, families, and external agencies, ensuring that everyone involved receives timely and appropriate information.

You will have knowledge and understanding of safeguarding principles and of Metal Health Capacity legislation, along with the capability to provide guidance that supports the resilience of families. Maintaining professional boundaries while fostering a supportive environment is a key aspect of this position. 

Driving Licence and use of car with ability to travel across the catchment area as post requires is essential.
