Job overview
An exciting opportunity has arisen to join Lancashire South Cumbria Foundation Trust’s Central + West Network to work as a Specialist Occupational Therapist.
The role will be predominantly in the Home First Team, made up of physiotherapists, occupational therapists and technical instructors. With a focus on supporting discharges from the acute hospital by providing a therapy assessment of patients function and care needs in their home environment following the principles of discharge to assess model.
The Home First Team works collaboratively with the CATCH (central allocation for care and health) team which is a multidisciplinary integrated health and social care team working together to ensure the most appropriate intermediate care services are commissioned based on the patient’s needs.
Main duties of the job
You will improve experience for patients and their families in relation to the discharge process; promote continuity of care from the acute into the community by supporting an integrated approach. Enable patient’s to recover within their home environment after assessment of needs within their usual place of residence.
In this role there maybe times where you are asked to cover across the wider community network which may include the RAPID assessment team as service needs indicate.
Within the Specialist Occupational Therapist position part of the position will be to undertake clinical triage supporting timely discharges from acute ensuring the most appropriate services are commissioned and also to support admission avoidance triage.
We are looking to appoint dynamic, enthusiastic and forward thinking Occupational Therapists to join our service. With excellent communication skills who can demonstrate sound assessment skills and has the ability to work autonomously as well as part of a team.
Hours may be required to be worked over 7 days between 08.00am and 08.00pm, depending on service need.
