Job overview

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

An opportunity has arisen for us to recruit a motivated and experienced Social worker to form the core of our countywide review service which will be based within the Community Mental Health Service. The post holder will have a responsibility to act as the Key worker for a caseload of service users who are placed within Cornwall and are in receipt of an adult social care and /or ICB funded package of support. 

This role will be essential in ensuring that this group are supported to receive the funded package, that providers are meeting their contracted requirements to deliver high quality and that resources within CFT are utilised properly in support of these service users. 

The post holders will work closely with our colleagues within the CMHT’s and Adult Social Care to ensure regular and timely review are undertaken.

Main duties of the job

You will be undertaking a range of social work tasks as required within the team or specialist service including but not confined to the following:

  • As part of MDT, participation in related assessments eg. HCR-20, CTR, facilities assessments
  • S.117 Care Plans, other Support Plans
  • Social Supervision on behalf of the Ministry of Justice (where applicable)
  • Personal and Family History assessments and Social Circumstances Reports
  • Attendance at Mental Health Tribunals to give evidence.
  • Create/contribute to evidence-based risk assessments and risk management plans, both longterm and immediate
  • In-depth and complex Mental Capacity Act 2005 assessments, Best Interest decision-making
  • Applications to the Court of Protection (e.g. regarding deprivations of liberty in the community)
  • Identify appropriate residential care to meet the patient’s needs (liaising with a range of
    commissioners and other professionals where necessary)
  • Support access to Advocacy services including IMHAs, IMCAs and Care Act advocates.
  • Travel out of county to provide social work input for service users in placements OOC – this
    may occasionally involve overnight stays OOC.
  • Lead on and develop the provision of support for Family and Friends
  • Complete regular Carer Questionnaires and use result to adapt and improve provision.
  • Support carer involvement, where appropriate (including facilitating their attendance at
    relevant meetings as needed)
  • Create Friends and Family Passport
  •  Provide information and signposting

Essential criteria
  • Qualified Social Worker
  • Registered Social Worker with Social Work England
  • Experienced in delivering care/support to adults with mental health issues
Desirable criteria
  • Knowledge of benefits/employment systems
  • Awareness of local services

Work Experience

Essential criteria
  • Knowledge of the Mental Health Act 1983 and Mental Capacity Act 2005
Desirable criteria
  • Experience of supervising/managing people
  • Additional specific professional training (e.g. in Autism, Safeguarding, HCR-20 risk assessment)
