The post holder will be responsible for ensuring that the needs of those assessed who meet the eligibility for CHC are being met appropriately and proportionally by high quality commissioned care. This will include managing a team of CHC assessor and provide clinical support to both clinical and non-clinical staff including the commissioning team, procuring and providing care packages in a manner which reflects the choice and preferences of individuals whilst balancing the need to commission care that is safe and effective and makes the best use of available resources. The role will cover All Age Continuing Care:

 • Learning Disability 

 • Physically and or Sensory Disabled

 • Organic Mental Health

 • Palliative Care

The post holder will be a member of a senior management team responsible for ensuring that SWL CCG is fully compliant with:

 • The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care [revised July 2022]

• Personal Health Budgets.

• CHC Operating Plan and Quality Assurance Framework.

• Court of Protection Deprivation of Liberty

 

Main duties of the job

The post holder will be required to be proficient in the analysis and interpretation of Continuing Healthcare data, strategic planning, specifying outcomes and procuring care, managing demand and performance for all services that are required to meet the needs of all individuals who qualify for NHS Continuing Healthcare. 

The post holder will support to develop and lead on some of the key areas of service development, quality, monitoring, financial rigour and commissioning, working with clinical leaders and partners, to support the delivery of CHC, FNC and PHB across South West London.

Support the Deputy Director of Continuing Care in providing expert clinical leadership and advice to the ICBs AACC team and wider health and social economy and partners.

Duties and Responsibilities

To provide effective clinical leadership to clinical teams & CHC team members, enabling the team to deliver to the highest possible standards, ensuring that evidence based practice is used when completing assessments and commissioning individual care packages.
• Working in accordance with the CHC Operational Policy, to ensure completion of referrals and reviews.
To monitor effectiveness of internal ways of working and develop action plans as required for improvement/gaps in system effectiveness.
• Ensure timely assessments and reviews are undertaken by the team to determine the eligibility of CHC funding in accordance with the National Framework.
• The management of all eligible cases, ensuring systems are in place for effective reviews.
• Ensure timely reviews are taken by appropriate clinical staff in relation to out of area CHC placements
• Ensure that eligible cases have a personalised care and support plan in place through the offer of Personal Health Budgets.
• Review complex cases to ensure that care plans are clinically appropriate to meet the assessed need of the individual through the provision of high quality, value for money packages of care.
• Timely verification of the assessment ensuring compliance and a consistent process with the National Framework
• Line management of the nursing team and provide peer supervision.
• Provide clinical support to the commissioning of care packages from independent providers ensuring safe care is delivered
• Responsible for timely progression of the appeals process to ensure compliance with statuary requirements.

Work with the Deputy Director of Continuing Care to develop and implement a joint NHS and LA CHC and PHB training programme across the health and care economy
• Support and deputise for the Deputy Director of Continuing Care as and when required.
• To support the Deputy Director of CHC in legal challenges and maintain awareness of test cases and case law that may have implications for CHC practice ensuring cascade and communication to key members of the team and the wider health and care economy
• Provide expert clinical advice and leadership to the ICB CHC case management team
• Ensure the flexibility of services if required to meet conflicting/changing requirements.
• Support the Head of Continuing Care in ensuring that all aspects of enquiries, compliments, disputes and complaints are investigated, managed and co-ordinated in a timely and appropriate manner and deliver a satisfactory outcome.
• Fully engaged in compliance with Safeguarding Vulnerable Adults Policy and Procedures and work in partnership with both LA and ICB Safeguarding Adult Leads
• To be knowledgeable and understand the, Deprivation of Liberties and Mental Capacity Act ensuring timely identification and applications of Court of Protections are made.
• Represent the ICB at IRP at NHSE London
• Ensure the delivery of PHBs and ongoing reviews

Person specification

Education /Qualifications

Essential criteria
  • Registered Nurse Qualification
  • Master’s Degree or equivalent level of experience in health or social care
  • Evidence of professional development

Knowledge and Experience

Essential criteria
  • Relevant nursing experience gained in a complex, multi-disciplinary environment
  • Significant experience in CHC and operating in a politically sensitive environment
  • Implementation understanding of national policy and continuing care framework

Skills and Abilities

Essential criteria
  • Excellent written and verbal communication including providing and receiving complex, sensitive or contentious information
  • Comfortable with complex and demanding workloads and competing priorities
  • Demonstrable ability to deliver the remit of the post
