The Transformation Manager for Learning Disability Services is a pivotal role responsible for leading the transformation of community-based specialist health services for adults with learning disabilities across Berkshire. The post holder will work closely with the Senior Leadership Team to oversee the development and implementation of evidence-based clinical pathways, ensuring high-quality, person-centred care that is responsive to local needs.

We are seeking a clinical leader with a strong background in learning disability services and experience in managing complex projects. The ideal candidate will have excellent communication and organizational skills, the ability to build relationships with stakeholders, and a commitment to delivering innovative solutions that improve service delivery and outcomes for people with learning disabilities.

Prif ddyletswyddau'r swydd

  • Oversee the transformation and "Reimagining of specialist community health services for adults with a learning disability" across Berkshire
  • Develop evidence-based and consistent clinical pathways
  • Lead on arrangements for transitioning into operational practice
  • Monitor the realisation of benefits within clinical service budgets
  • Provide strong clinical leadership, support, and guidance through a range of project-related activities
  • Work closely with people using our services, their families/carers, inpatient and community-based teams, primary care, other health and social services, and third sector organisations
  • Develop relationships with key stakeholders across BHFT localities.
  • Lead the specification and assessment of clinical changes proposed under the project
  • Plan and lead workshops and events as required
  • Work with the Associate Director of Learning Disability Services and the Senior Leadership Team to develop and project the vision, values, and objectives of the learning disability services
  • Identify clear benefits, goals, and processes for developing ideas, knowledge, and work practices
  • Represent the service on local operational and strategic partnership groups and participate in multi-agency planning for people with learning disabilities
  • Lead on the delivery of clinical elements of the project which will be high profile and highly complex in nature

The “must haves” for this role:

1.    Registered Health Professional: You must be a registered health professional.
2.    Experience in Change Management: You will have experience in change management, demonstrating the ability to implement innovative clinical practices and manage transitions effectively
3.    Experience in Learning Disability Services: You will have at least 5 years of post-qualification experience, including recent experience in learning disability services.
4.    Excellent Organizational Skills: You must possess excellent organizational skills, with the ability to plan and prioritise your workload efficiently

Hours: 37.5 hours per week (part-time hours and job share can be considered)
Contract Type: Fixed Term - 2 years.
Location: Based in Reading, with Trust-wide responsibilities.
Flexibility: Opportunities for home and flexible working arrangements – although travelling to sites in Berkshire will be required at times.
