An exciting opportunity has arisen for an experienced Speech & Language Therapist with advanced clinical skills to be part of the established and expanding Community Neurological Rehabilitation Team (CNRT) in Central Lancashire. The CNRT provides ongoing rehab to patients within the Chorley, South Ribble and Greater Preston locality with predominantly Stroke and ABI diagnosis. Referrals to the team are mainly from the acute hospital, inpatient setting, however referrals from the third sector are also accepted.
The team has an active involvement with the Lancashire & South Cumbria Integrated Stroke and Neuro Rehabilitation Delivery Network and is currently working towards the implementation of the Lancashire and South Cumbria Stroke Service Specification.
We are looking for an enthusiastic, versatile, innovative individual with a commitment to working within an interdisciplinary environment, who has experience of working with patients with stroke and neurological conditions. You will be offered regular supervision by a highly experienced therapist to support you to develop and expand on your existing skills and knowledge. You will be responsible for the assessment, treatment planning and management of rehabilitation programmes for a wide variety of clinical presentations and will be responsible for delegation to generic rehabilitation assistants and support them in the delivery of this rehabilitation. You will have autonomy in managing your caseload and diary.

To be responsible for the assessment, treatment, planning and delivery of evidence –based rehabilitation interventions.
To work as part of the rehabilitation and or stroke/neurological service line and be responsible for assessment and management of a clinical caseload of patients with complex needs.
To undertake comprehensive assessment of patients with complex presentations, using investigative and analytical skills, to formulate an appropriate hypothesis and to determine a diagnosis and appropriate management plan.
To establish individualised management and treatment plans, using clinical reasoning and utilising a wide range of evidence based treatment skills and options to formulate highly specialised interventions and programmes of care, which may include working with staff across organisational boundaries.
To promote health and reduce inequalities for all patients accessing the service, so that they may be able to maximise their potential in terms of health education and social well-being.
Contribute to clinical leadership and supervision facilitating a culture of learning and reflective practice within podiatry.
Document and evidence patient outcomes.
To lead/contribute to training and development programmes as appropriate to the knowledge and skills of the individual, delivering to other professionals both within and outside the NHS and service users.

- Current professional registration with HPC as a speech therapist and responsibility for maintaining this Registration
- Evidence of CPD in related field of clinical expertise.
- Post graduate qualification in specialist field

- Understanding of clinical governance framework and implications for practice including audit and research governance.
- - In-depth understanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents.
