Job Description Mental Health Practitioner
Grade Agenda for Change Band 5
Based at Avondale Unit Royal Preston Hospital
This is an exciting and new opportunity for a Registered Mental Health Nurse to join the Home Based Treatment Team within the Central locality on a developmental post, the successful candidate will start as a band 5 and works towards achieving a band 6 within 12months.
Successful applicants will be provided with a full induction to the team, will be provided with an induction pack outlining our service and ways of working, and will have regular clinical supervision sessions and an opportunity to complete their appraisal focussing on their personal and career development. Furthermore the successful candidate will have the opportunity to complete the in house bespoke development training package to ensure they are fully supported and achieve the competencies as required to become a band 6 practitioner within the CHBTT.
Main duties of the job
1. To deliver high quality care to individuals who have specific mental health problems i.e. have a severe and enduring mental health problem and multiple, complex needs.
2. To act as a key member of the Multi-Disciplinary Team ensuring the effective assessment, including Risk Assessment and Risk Management, planning monitoring and evaluation of care giver to service users with severe and enduring mental illness in acute crisis, ensuring the effective supervision of staff accountable to the post holder, whilst liaising with a wide range of multi-agency professionals
3. To provide a “gatekeeping” function as detailed in the Operating Procedure.
4. To provide rapid and holistic assessment, care and sign posting of individuals within the above group but also for those presenting with other urgent risk issues.
5. The work of the Crisis Team will be managed as a whole though the post holder has crisis co- ordination responsibilities for a caseload specified within the team
6. To act in the role of “Shift Co-ordinator” as detailed in role protocol.
7. To undertake the role of Bed Management for the local area.
