West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

I’m excited to offer this opportunity to a forward thinking and proactive Registered Nurse for people with learning disabilities (RNLD). This role is ideal for a newly qualified RNLD or an RNLD that wishes to work within the community setting.

Our central focus is to have the Service User and their family at the core of what we do. We are deeply committed to delivering on an equality agenda that delivers in reducing the health inequalities experienced by this population group and improve health outcomes. 

In 2021, the service was nationally recognised and received a ‘highly commended’ accolade from the National Pharmacy Awards. In 2022 we reached the finals at the National Learning Disability and Autism Awards and later that year received the Trust’s ‘Outstanding Clinical Team of the year. We are rightly proud of what we have achieved so working in innovative ways is the standard we always aim for! 

 

Prif ddyletswyddau'r swydd

You will work as an integral member of the Team whilst having responsibility for a designated caseload (with support/steer from higher banded nurses). The post holder will be expected to positively represent the needs and rights of people with learning disabilities, tackling inequalities in health, and improving access to identified services in the statutory and 3rd sectors

You will work in partnership with generic health services, service users and carers to support the implementation of Health Action Planning, supporting practice nurses, GPs and other primary and secondary care services (including mental health) to undertake this

You will participate in undertaking depot clinics (and home visits) as required, and to administer the depot medications within the knowledge and understanding of uses, including potential side effects

Qualifications

  • Bachelor’s degree or Diploma (related to nursing/health)
  • Live registration with the NMC
  • A sound understanding of legislation and policy as it relates to people with learning disabilities

  • Professional associations and special interest groups as they relate to people with learning disabilities

Experience

  • Experience of/ability to demonstrate working in a multidisciplinary team (within and across organisations)
  • Experience of working with people who have mental health difficulties and those detained under the Mental Health Act
  • Sound knowledge and experience of Liberty Protection Safeguards (LPS) and mental capacity assessments and wider practice
  • Experience of working with acute physical and mental health service providers

  • CPPD approved assessing/mentoring course
  • Experience of clinical audit

Knowledge

  • Knowledge of IT systems e.g. PowerPoint, Excel and other associated programmes

  • Good understanding of the mental health needs/issues/challenges of people with learning disabilities
  • Robust understanding of the diverse physical health needs/issues/challenges of people with learning disabilities
  • Understanding of the Mental Health Act and how that works in practice
