Ormskirk Older Adult Integrated Mental Health Team, are seeking an experienced Band 4 Health Care Support Worker to join the team. This is an exciting opportunity and the successful candidate will be part of an evolving service that has the needs of the older adult population at its heart. We are looking for someone who is enthusiastic and passionate about older adults to come and join a forward thinking, dynamic and constantly developing team. 

The team provides high quality care working with older adults with dementia or significant functional / psychological mental health needs, ensuring they have access to timely, intensive and specialist mental health care. We also work with families and carer’s and alongside other agencies to promote well-being and support for individuals and their carer’s / families. 

The team covers various functions, Home Treatment, Care Home Liaison, Community Mental Health Team and Memory Assessment. The Health Care Support Worker is a valued resource within the team and works closely with all other team members. The applicant must have experience of working with older adults with dementia and be able to demonstrate a working knowledge of older adult mental health and the needs of individuals in this group. You will have excellent clinical skills along with outstanding communication skills

The post holder will work as a member of the Older Adult Community Mental Health Team and provide assistance to qualified staff within the team in undertaking their duties. They will provide a high standard of care to patients as appropriate to support their mental and physical health needs through their journey within the team.
You will be an advanced health care support worker working within a multidisciplinary team and carry out a diverse range of duties.
The post holder will assist in the delivery of the objectives set out within the Lancashire and South Cumbria NHS Foundation Trust business plan.
You will be required to take on tasks that require you to operate electronic care records using the RiO systems and staffing record systems such as PDR/supervision and be compliant with all mandatory training.

You will maintain a positive and open attitude towards organisational change and service development.
