Job overview
Are you the nurse we are looking for to join our team? As a Practice Nurse you will be afforded the opportunity to join the primary care team in
Canolfan Goffa Ffestiniog working towards the delivery of a new model of multi-disciplinary health care services to the people of Blaenau
Ffestiniog and surrounding areas.
This dynamic Team currently comprises of Advanced Nurse Practitioner, Primary Care Practitioners, Practice Nurses, Assistant Practitioner,
Advanced Pharmacists and Physiotherapist in addition to GPs and other allied health professionals.
You will undertake a wide and interesting range of Nursing duties relevant to primary care and the population needs including wound care,
immunisation, contraception and sexual health services, cervical screening, warfarin monitoring, lifestyle advice, assisting with minor surgery, travel health and long-term condition management. You will work closely with the wider managed primary care practice teams in the west of BCUHB. The successful applicant should already have experience of long-term conditions and working in Primary Care or General Practice .
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
The purpose of the post is to provide nursing services to the patients of the practice. The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and within defined competencies.
The main duties of the role involves undertaking chronic disease management clinics to support the Quality and Outcomes
Framework (QoF) including Asthma, COPD/Spirometry, Diabetes and Chronic Heart Disease. Identifying, and managing as appropriate; treatment plans for patients at risk of developing a long term condition, performing basic nursing tasks such as ear syringing, blood pressure monitoring, ECGs etc, assessing and caring for patients presenting with wounds including the removal of sutures and clips and leg ulcer management, participating in vaccination and immunisation programmes; administering injections, ensuring safe vaccine storage and implementing cold chain policy, undertaking cervical cytology screening on those who are eligible in line with national guidelines, undertaking health screening, surveillance and new-patient screens, take pathology samples from patients for analysis (blood, urine etc), assisting GPs with minor surgery procedures and assessing problems presented opportunistically by patients, dealing with minor illness as well as in minor illness clinics.
