Sirona Single Point of Access (SPA) is looking for an Advanced Practitioner who shares its vision to drive equitable "Patient Centred" health care to adults within the BNSSG community by providing knowledgeable, efficient, compassionate and respectful remote advice and support." 

SPA work closely with system partners to deliver new ways of working such as Frailty ACE and pathways from SWAST to Urgent Community Response.  We are looking for a partner to support us, continuing to change the way we work.  The role will clinically lead and champion remote clinical decision making, promoting positive risk taking.  Within the role there will be opportunities to develop your leadership skills, share your knowledge and passion for advanced practice, whilst also having opportunity to help shape and develop the service and your role within.  

SPA is a fast paced and dynamic environment operating between 0730 and 2000, 7 days a week.  The service is comprised of a Clinical Team with a diverse multidisciplinary background working alongside a large group of Heath Navigators who provide frontline advice and routing of service users accessing SPA.  As a service we embrace hybrid working coupled with flexible working patterns and the use of digital innovation to deliver and improve the service.

Main duties of the job

We are looking for a candidate who is able to demonstrate and apply advanced skills in all four pillars of advanced practice.

Clinical Practice

  • Confident non-medical prescriber, working autonomously in the management of a range of clinical presentations alongside experience of delivering these remotely.
  • Remote assessment of patients with urgent care needs using an array of digital tools
  • Work as a point of escalation to support more junior staff to manage a range of patient presentations

Leadership and Management

  • Leadership and management of Sirona's part in delivering the BNSSG system collaborative service Frailty – ACE (Assessment and Coordination of Urgent/Emergency Care).
  • Leadership and management of SPA SWAST pathway 

Education

  • Lead on workforce development with supporting education and training across SPA.
  • Lead and oversee the development of competency frameworks.

Research

  • Leading clinical excellence through application of audit and research to demonstrate evidence-based practice. 
  • Actively identify potential need for further research to strengthen evidence for best practice. This may involve acting as an educator, leader, innovator, and contributor to research activity and/or seeking out and applying research funding.

Qualifications

Essential criteria
  • Registered Health Care professional NMC/HCPC
  • Masters level 7 Advanced Practice/completed Advanced Practice Pathway
  • Independent Prescribing Non-Medical Prescribing Qualification
Desirable criteria
  • Recognised post graduate management / leadership qualification, working towards one or willingness to undertake.
  • Experience of developing advance practice guidelines, governance and audit

Experience

Essential criteria
  • Excellent communication skills with MDT members, patients and their families
  • Demonstrable experience of working at Advanced Practice level within your clinical area
  • Clinical experience in primary care and secondary care referral processes
  • Management of frail and elderly patients and the associated medical complications
  • Development in professional and management procedures within own area and across several related disciplines relevant to service area.
Desirable criteria
  • Experience of working at Advanced Practitioner level within a health care setting
  • Operational management experience including experience of managing and developing multi-disciplinary or integrated pathway teams in a variety of locations.
