Job overview

We are looking for a Senior Diabetes Specialist Nurse to join our team to support a complex caseload of all types of diabetes as well as offering advice, guidance and support to primary and community care via virtual clinics, education and advice and guidance

We work closely with other health providers such as Primary Care, Acute Hospitals, Ambulance service. We have a strong focus on collaborative working to enable knowledge and skill development. The role includes the delivery of education sessions to health care professionals and the delivery of structured patient education across BNSSG for type 1 and 2 diabetes  We are looking for someone who has a passion to improve Diabetes health outcomes across BNSSG. Alongside the clinical aspect of this post, you will have the opportunity to participate in audit, research, pathway and policy development, staff training.  You will demonstrate good leadership and team working skills with a practical, forward thinking approach to service delivery and improvement. You will be an excellent communicator with the ability to build relationships with a variety of stakeholders. 

The service is committed to CPD with good clinical supervision models and in house CPD in place to support you, including regular team MDT sessions.

 There will be an element of working across BNSSG to meet service delivery.

Main duties of the job

 

The post holder will deliver high quality care within the diabetes pathway and provide expert clinical practice, advice to patients and support to staff. This is delivered in a range of community and primary care settings. 

 

This will include providing clinical expertise, underpinned by the advanced skills within their speciality developed in Clinical Practice, Facilitating Learning, Leadership, and Evidence, Research and Development

Taking a key role in the development and delivery of the Diabetes and Nutrition Service (DANS):

 

  • Through an Expert Clinical Diabetes role in conjunction with the Clinical and Operational Leads within the service
  • Working collaboratively with other members of the multi-disciplinary team and key stakeholders to establish a whole systems approach in order to deliver improvements  in healthcare for people with diabetes
  • To develop and deliver Health Care Professional training and Structured Diabetes Education to people with diabetes

Experience

Essential criteria
  • Advanced Clinical skills and experience of working with people with diabetes
  • Able to demonstrate leadership skills, building effective working relationships with staff and management at all levels within the organisation and external environments;
  • Exceptionally efficient, with excellent prioritisation, time management and organisational skills and the ability to work flexibly and on a number of projects at one time to tight deadlines and at time under pressure;
  • In-depth experience of managing staff (of own and other professions) including providing feedback and assessment, developing skills and competencies and delegating work as appropriate;
  • Ability to travel throughout service delivery geographical area
  • Experience of working across organisational and professional boundaries to improve care for people with long term conditions
Desirable criteria
  • Master’s level clinical qualification or working towards achieving this.
  • Coaching / Teaching and Assessing qualification / experience
  • PADRAP/PACR

Qualifications

Essential criteria
  • Current registration with NMC / HCPC or other relevant professional body of registration
  • Successful completion of a relevant post-graduate L3 (or higher) course or appropriate equivalent modules, such as disease specific qualifications
  • Independent prescribing
  • Hold a UK Driving License and have access to a vehicle or transport during working hours.
