Job overview

The Sirona Adult Learning Disability Health (ALDH) Service is a centre of excellence for specialist learning disability health needs.

We cover 6 localities where multidisciplinary teams enable our service users to access health services through direct care, liaison with other services and offering support and training on how to work with people who have a learning disability and how to make reasonable adjustments. The service prides itself on delivering high quality support to service users with a variety of physical and mental health issues alongside their Learning Disability.

All staff are required to use a range of communication and listening skills to work effectively with our service users.

Where necessary you will be required to work at other bases or work remotely from home.

Main duties of the job

You will support the Services Manager to provide overall strategic management, business planning and development of the BNSSG-wide Adult Learning Disability Health Service (ALDHS). 


You will lead the Health Facilitation team within the ALDHS, working collaboratively with your Clinical and Operational Lead colleagues within other functions of the service and with senior clinicians in the planning, provision, improvement and development of services. 


The Health Facilitation team provides specialist learning disability support encompassing:

  • Direct care to patients including providing personalised planned interventions to support people to remain safe, well and as independent as possible in their own homes;
  • Education and training to health care professionals across the pathway  
  • Developing the focal point for responsive, high quality clinical advice in the out of hospital system, closely integrated with primary and secondary care specialties ensuring a seamless service.
  • Promoting self-care, providing good quality patient information and encouraging peer led improvements
  • Delivering service improvements resulting from key themes from LedeR, as well as other projects and training to improve the health outcomes for the learning disability population.
  • Facilitating access to acute hospital care for people with a learning disability and / or autism to the acute hospitals within the BNSSG (acute liaison team)

Skills and experience

Essential criteria
  • Previous experience working in a relevant specialist service setting and/or other relevant multi- disciplinary environment, of a similar size and complexity.
  • Able to demonstrate a good understanding of the transformation agenda and the legislative, regulatory and performance management framework for health and social care.
  • Evidence of ability to resolve complex issues within service, including with staff implications using sound logic and taking into consideration relevant business implications.
  • Specialist knowledge of the relevant Specialist pathway and services and demonstrable good understanding of several related disciplines
Desirable criteria
  • Involvement in strategic decision making and partnership working
  • Experience of working at National or regional area on steering groups

Qualifications and training

Essential criteria
  • Professional qualification in Health Care and current NMC registration.
  • Operational management experience including experience of managing and developing multi-disciplinary or integrated pathway teams in a variety of locations.
Desirable criteria
  • Recognised post graduate management / leadership qualification, working towards one or willingness to undertake.
