We are seeking a Registered Pharmacy Technician to join our Medicines Optimisation team!

The Medicines Optimisation team support people in taking their medicines safely and effectively. The aim of the service is to improve individual outcomes by optimising medicines use. The Pharmacists and Pharmacy Technicians are also available to healthcare professionals for advice on specific individual medication issues, particularly for complex individuals.

You will work with the team to support people in the inpatient setting and at home. You will play a key role in supporting the safe transfer of care between care settings, preventing readmission for people recently discharged from hospital, supporting people to regain independence with medication and recommending short or longer term options to aid compliance. 

 

Main duties of the job

We are looking for a registered pharmacy technician to work at South Bristol Rehabilitation Unit, the unit consists of 60 beds, 30 of which are Subacute Stroke Rehabilitation beds and 30 general Rehabilitation beds.  

Working closely with all members of the multidisciplinary team, your responsibilities will include medicines reconciliation for newly admitted service users, counselling on the use of medications, ordering and supplying medicines, facilitating discharges to ensure safe transfer of care, supporting the delivery of the medicines optimisation audit programme and contributing to governance arrangements.

You will also support ‘self-administration of medication’ across both units. This will entail ensuring that all patients are assessed for suitability on admission to the unit; evaluating and optimising the suitability of medicines for self-administration; and educating and supporting other staff groups in the safe implementation of the policy. 

You will be line managed by the lead medicines optimisation technician, working closely with the unit’s clinical pharmacist and be supported by the wider Medicines Optimisation team. You will be a valued member of the established multi-disciplinary and multi-agency team on the Rehabilitation Units and will be supported by them to deliver person-centred care.

Qualifications

Essential criteria
  • BTEC or NVQ 3 in pharmaceutical services or equivalent
  • Registered as a pharmacy technician with the General Pharmaceutical Council (GPhC)
  • Accredited Medicines Optimisation / Medicines Optimisation Technician – SW regional qualification or equivalent
Desirable criteria
  • Evidence of continuing professional development (CPD)

Experience

Essential criteria
  • Previous medicines optimisation experience including assessment of patients own drugs and medicines reconciliation.
  • Experience of working with partner organisations.
  • Experience of policy and procedure implementation.
  • Experience of stock control.
  • Experience of audit techniques.
Desirable criteria
  • Experience in Community Health Services or secondary care.
