Job overview

An opportunity has arisen in the Bristol Pathway 3 Therapy team for a Rehabilitation Support Worker who has a genuine interest in working in rehabilitation.

The successful candidate will be comfortable working independently in a care setting as well as working as part of a team; have excellent communication skills and be able to take the initiative to ensure that service users and their families have the best experience being supported by us.

You will support the therapy team in the management of service users who have been recently discharged from an acute hospital setting and are undertaking a period of assessment and rehabilitation.

You will be working within a friendly and supportive team, where we are committed to ongoing clinical development, education and further study. We will provide you with regular training and supervision.

The role will provide support to the out of hospital assessment beds located in local care homes and requires the flexibility to work across the bed bases in the Bristol area.

Main duties of the job

  • Promote optimum independence through encouraging rehabilitation techniques.
  • As a rehab support worker you will be a key figure in completing the day to day rehabilitation with patients under the instruction of the lead therapists. You will be required to provide feedback on the service users’ progress and help to ensure that the service users achieve their maximum potential.
  • Undertake a range of delegated clinical and therapy led activities to support the service users on the caseload.
  • To provide safe, patient centred, effective and evidence-based clinical activities.
  • The post holder will work under direct supervision and be responsible to senior therapists providing one to one and group rehabilitation.
  • You will work within the Adult Service to deliver a full range of health and social care activities, both acute and long term, supported by a robust competency framework to adults within the complex assessment beds.
  • You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, physiotherapists, occupational therapists and other Allied Healthcare Professionals 
  • The Service operational Hours are between 08:00 and 18:00 (flexible working can be discussed) with future possibility for weekend cover.

Essential criteria
  • NVQ level 3 in Health & Social Care or willingness to work towards
  • Numeracy and literacy level 2 or equivalent
Desirable criteria
  • Apprenticeship in Health (level 3)

Skills, Knowledge & Experience

Essential criteria
  • Able to work as part of a multidisciplinary team but also to act on own initiative
  • Effective communication and interpersonal skills
  • Experience of direct patient care in a health or social care setting
  • Full driving license and access to car in work time
  • Ability to use a range of equipment to aid independence
Desirable criteria
  • Recent relevant experience within a rehabilitation setting
