Job overview

The post holder will be a member of the Palliative & End of Life Care Nurse Facilitation Team, working within the North Wales Department for Specialist Palliative Care.

The post holder will contribute to the delivery of a high quality, accessible palliative care service, providing specialist support, training and education for palliative and end of life care.

They will be responsible for the coordination and delivery of the work and education streams for palliative and end of life care.

The post holder will complement the work of the acute and community nursing services providing palliative care training and education.

The post holder will be expected to work flexibly and rotate across North Wales communities as required to cover the service.

Main duties of the job

We are delighted to offer this exciting  opportunity for an appropriately experienced and qualified nurse to work as part of the award winning Macmillan End of Life Care Nurse Facilitation Team to provide facilitation and support for the implementation of tools and training required to support high quality end of life care in a variety of settings, in North Wales. The post holder will work with the support of the Lead Nurse Facilitator for End of Life Care and the Specialist Palliative Care Team, working across a variety of settings, including,  Care Homes, Community Hospitals, primary care and secondary care.

The post holder will support facilitation of end of life care programmes (Six Steps to Success), facilitating delivery on key aspects of implementation, development and evaluation of the strategic work streams. This role requires effective communication, co-ordination and negotiation skills which are essential to work in partnership across organisations.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
