This is an exciting opportunity for an Assistant Practitioner to work as part of our multi-disciplinary Integrated Network Teams mainly working within the therapy team 

By joining us you will have many opportunities to develop your clinical skills as well as physical assessment and clinical reasoning with the support of the wider team.

Main duties of the job

You will work as a member of the team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.

You will:

  • Provide assessment and treatment to patients on the caseload with the guidance of the wider team
  • Maintain a high professional standard of care in adherence to all protocols/policies & guidelines
  • Manage a patients care plan within your scope of practice and with the support of the wider team
  • Provide clinical assistance to other members of the clinical team 
  • Maintain accurate electronic clinical records

You will work within our service that is delivered from 8am-8pm, 7 days a week service.

You will be required to respond to referrals across urgent and planned pathways including hospital discharge facilitation.

You will work as part of the  team to proactively monitor and manage patients on the caseload to ensure the best outcomes for them.
