Do you want to broaden your experiences, completing specialist risk assessments and treatment interventions across secure hospital and community settings?

There is an exciting opportunity for an experienced Learning Disability nurse to join the Secure Outreach Transitions Team.  Based at Roseberry Park Hospital the service operates across Teesside, Durham, Darlington and Redcar & Cleveland.  The team provides support to service user’s and care teams 7 day’s a week over an 8-8 period

The service provides secure care and treatment to individuals with highly complex clinical presentations in community and in patient settings.  

We work with those who are diagnosed with a Learning Disability with/ without Autism diagnosis who pose a significant risk to others within the community. Additionally, we support individuals who are admitted to Secure Services and work with inpatient clinical teams to reduce length of stay. The team also works alongside secondary mental health and learning disability services, offering comprehensive risk assessment, shared care and CPA management responsibilities across the Tees, Esk and Wear Valley’s NHS Foundation Trust area.

The successful candidate will have the opportunity to develop, therapeutic interventions, training and group-work.

Main duties of the job

Delivering high quality patient care 

Successfully communicating with others, establish and maintain great working relationships and gain co-operation

Effectively analysing information, identify problems, develop solutions and make recommendations/decisions

 Effective processing and management of information and the use of information

Developing and implementing policies and or services/systems

Managing finance and physical resources/assets e.g. equipment, fixtures and fittings, stationary etc

Undertaking research, audits and governance, providing assurance to others

The Secure Outreach Transitions Team is a multidisciplinary service based at Roseberry Park Hospital in Middlesbrough.  The team has close links to the Secure Inpatient Service, local community mental health and learning disability teams and private healthcare providers across the region

Qualifications/Experience

Essential criteria
  • Current Professional Registration with Nursing and Midwifery Council - RMN / RNLD/ RGN
  • Recognised Practice Assessor/Practice Supervisor or currently in progress of achieving.
  • Trained in Clinical Supervision and providing Clinical Supervision or in the process of achieving
  • Postgraduate diploma in a subject relevant to the area Or be willing to work towards and achieve within 2 years and
  • Within the first year of being in post hold a recognised qualification in a governed therapy/advanced clinical qualification e.g Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy /NMP (Independent Prescriber)
  • And have Equivalent clinical experience to that of a Post Grad Diploma/master’s level such as working as a band 6 (or above) in relevant clinical area with significant experience (see below under experience), together with evidence of relevant CPD

  • Significant experience working with patients with complex needs in the specialist field.
  • Significant experience working with patients who display risk behaviours
  • Clinical risk assessment and management and its application in practice
  • Working collaboratively with service users their families and carers
  • Working in a multi-disciplinary team
  • Quality improvement activities
Desirable criteria
  • Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally
