We are looking for a motivated, compassionate and ambitious Nurse who have a genuine interest in working with elderly service users and delivering high standards of care.  We are looking for a night Nurse who is wanting to provide our patients with the best experience and treatment possible and are ready for the challenge of helping us to further develop our rehabilitation services.

Henderson Rehab Unit is based on the top floor of the Grace Care Centre  and is at the heart of our Pathway 2 rehabilitation service. If you are a Registered Nurse who enjoys working with patients to improve quality of life and promote independence then this is an exciting opportunity to join us in our 19 bedded physical rehabilitation unit. 

You will work as part of our multidisciplinary team, all working together, to deliver individualised nursing and rehabilitation care to patients with the common goal of delivering the best outcomes possible for our patients.

Our patients present with a broad range of conditions including orthopaedics, frail elderly, post-surgery and long term neurological conditions and we work with them to help them attain the highest level of personal independence prior to discharge.

Main duties of the job

The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of identified service users and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.

The post holder will-

Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.

With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.

Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.

You will be expected to work weekends and bank holidays on a rostered basis. Although this post is predominantly nights there may be some opportunity to work some day shifts.

  • Effective communication skills written, verbal and no-verbal with the ability to adapt communication for individuals with communication difficulties

  • Able to make clinically reasoned judgements when working with a variety of conditions
  • Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff
  • Effective time management and caseload management
Desirable criteria
  • Experience working with a range of professionals in health or social care.
  • Experience in the community.
