The Bedfordshire Luton and Milton Keynes (ICB) is responsible for planning and allocating resources to meet the four core purposes of the Integrated Care System (ICS), namely: to improve outcomes in population health and healthcare; tackle inequalities in outcomes, experience, and access; enhance productivity and value for money; and help the NHS support broader social and economic development.
Within this context, as the ICB develops as a statutory organisation, a significant challenge will be to determine how it effectively supports and enables place-based and local partnerships to take necessary action that will improve population health outcomes and reduce inequalities.

An exciting opportunity has arisen for professional with knowledge of Continuing Health Care or a desire to develop knowledge of Continuing Health Care. Bedfordshire Luton and Milton Keynes Integrated Care Board (BLMK ICB) are looking for someone who is enthusiastic, compassionate, and innovative, to work with us to take our vision forward, focusing on increasing the efficiency of the ICB Continuing Health Care Team.
The post holder will work as part of an integrated ICB team, alongside system colleagues plus regional and national teams to help deliver the best value from Local and National programmes and services by providing clinical expertise to support The Continuing Health Care’s integration pilots and projects and to embed new clinical services to fully realise their potential.
Our ICB offers superb development opportunities for all staff, and this role is ideal for aspiring practitioners as you will be responsible for leading continuing care across key business areas.
This opportunity is for a Zero hours contract and will form part of our Bank staffing program.
Training will be given for candidates that meet the the specifications outlined.

To manage a case load of patients through the Continuing Healthcare process, utilising knowledge, and experience to complete holistic assessments and reviews both remotely and in person. Supporting our partners and the public with insight and application of the National Framework.
Additional expectations are to support the existing team with undertaking any CHC related tasks. e.g. Duty, Safeguarding, Care planning, Personal Health Budget referrals, requiring key skills of communication, prioritisation and organisation.

To support the current CHC team deliver CHC related activities across the BLMK system which includes Luton, Bedfordshire and Milton Keynes.
The role will include - but not limited to:
Completion of Check lists and DST's and Fast Tracks.
Work streams are both remote and face to face and will be supported by the CHC team that are based in our 4 locations across the area.

- • Demonstrable experience of working in a large complex organisation.
- • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
- • Demonstrable experience of assessing an individual’s care needs, for example care and support under part 1 of the Care Act 2014, or a primary health need as defined in the Continuing Healthcare Framework Decision Support Tool
- • Further training or significant experience in project management, financial management or supporting change management processes
- • Experience and understanding of evaluating and measuring the performance of health services.

The CCGs in Bedford, Luton and Milton Keynes have come together under one governing body and senior team referred to as BLMK ICB. We are an exciting new organisation putting the needs of communities at the heart of our commissioning arrangements, by working with local partners across health social care and the third sector.
The Complex Care department includes a wide range of services including those for older people and/or those at end of life and personalised care through the commissioning of care packages and placements via personal health budgets or continuing healthcare funding.
The post holder will play a key role in supporting the Continuing Healthcare (CHC) service across the region, working within the remit of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, to ensure an effective and competent level of service that consistently delivers an individual-focused service, ensuring the delivery of key quality outcomes and strategic objectives.
The post holder will be responsible for the delivery of patient centred assessments in order to identify any primary health care need which may result in eligibility for CHC (or associated funding stream) ensuring the national framework for NHS Continuing Healthcare and NHS-funded Nursing Care is implemented and complied with according to the principles and processes. Where appropriate, onward referral to other agencies may be required.
