Senior Mental Health Practitioner - Home Treatment Team - OPMH

Are you an experienced registered Mental Health nurse, Occupational Therapist or Social Worker looking to secure the next step in your career or to take on a new challenge within Older People’s Mental Health Services? If so, we are looking to recruit an experienced qualified Senior Mental Health Practitioner within the Older Adults Mental Health Team (Home Treatment). 

If you join us, you’ll be well supported by a dedicated team that strives to create a progressive, friendly and compassionate environment.

You will work with the clinical team and take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia.

You’ll need at least two years of post-registration experience and specialist knowledge of the care of older people with mental health problems and mental health crises.  

The role is based at Nicholson House, Maidenhead and your time in this role will be spent visiting clients in their homes in the local community so you will need a car to get to appointments. You will be required to come to the office occasionally for team meetings, training or for other service-related reasons as necessary, although currently we conduct most team meetings virtually this may change in future.

The service operates 7 days per week, so you will work shifts to cover the service hours of 9am-7pm, with some weekends and bank holidays as required.

  • To manage and be responsible for the mental health care of an allocated caseload of patients. 
  • To undertake comprehensive initial assessments in a range of settings following acceptance of referral, taking into account the persons physical, psychological, social and spiritual needs. 
  • To develop and implement an agreed plan of care with the service user and where appropriate, include the views of families and carers. 
  • To provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment (up to four times daily). 
  • To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families. 
  • To initiate and carry out the agreed treatment plan.
  • To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or CPA meetings.
  • To have knowledge of a variety of therapeutic interventions including short term problem solving, crisis intervention and basic CBT techniques.

The “must haves” for this role:

  • Registered Mental Health Nurse /Equivalent Professional Qualification.
