Our Critical Care Outreach Service is offering an opportunity for an experienced Critical Care Outreach Nurse to join the team, and undertake a Trainee Advanced Clinical Practitioner (ACP) development programme as a Band 7. Upon completion of the three-year programme, and successful completion of competencies, you will achieve Band 8a. You will be joining an existing team of Advanced Clinical Practitioners & Clinical Nurse Specialists to deliver patient-focused programmes of care, which address the complex and changing health and wellbeing needs of the deteriorating and critically ill patients across the organisation.

The ACP-CCOR role is a cutting-edge role in the field of Critical Care Outreach which crosses professional boundaries, undertaking traditional medical roles and leading inter-professional practice. It is a role model of excellence in the delivery of holistic care to patients. The tACP will be learning to assess and treat the many varied clinical cases; both acute and chronic.  The post holder will develop critical thinking in safe clinical decision-making processes and will learn how to deliver an advanced level of clinical practice within critical care. Working within local and nationally agreed protocols and guidelines, the tACP develop the knowledge and skills required across the Four Pillars of Advanced Practice.

The  tACP will develop generic and specialist knowledge and skills in Critical Care Medicine to provided healthcare autonomously and within a multi-disciplinary team with a focus on Critical Care through an established ACCP training pathway.

The post holder will be a 1st level registered practitioner. They will have experience in working within Critical Care Outreach. They will have excellent communication skills, and be able to liaise with patients and their families in planning their healthcare needs. They will be expected to provide evidence of their ongoing continuing professional development. 

Whilst in training the tACP will undertake the MSc in Advanced Critical Care Practice. As well as the academic pathway the tACP will follow the three year consultant-supported ACP training program mapped against established national and regional competencies. The tACP will gain the skills & knowledge under direct consultant supervision and supervision of the Clinical Lead to undertake clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions.

The post holder will demonstrate critical thinking in the safe clinical decision-making process and is required to deliver an advanced level of clinical practice within critical care. Working within local and nationally agreed protocols and guidelines, the ANP demonstrates the knowledge and skills across the Four Pillars of Advanced Practice.

Clinical

Practice in compliance with their respective professional bodies (NMC) Code of Professional Conduct being responsible and accountable for their decisions, actions, or omissions.

Be able to demonstrate a critical understanding of their broadened level of responsibility and autonomy. This includes being critically aware of their limits of their own competence, including when working with complexity, uncertainty and incomplete information.

Independently prioritise, organise and manage a highly complex patient caseload.

Demonstrate a high degree of proficiency and clinical acumen in relation to decision making skills, to inform clinical reasoning approaches when dealing with differentiated and undifferentiated presentations and complex situations; synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses, treating, discharging, referring to alternative specialities or escalating cares as required.

Use own professional judgement to manage risk appropriately, prioritising the safety and wellbeing of individuals, families and carers.

To deliver an advanced level of clinical practice within the field of Critical Care Outreach and Hospital Out of Hours (HOOH) services as part of the 24/7 team. This autonomous role will exercise independent judgement to assess, investigate, plan, implement and evaluate clinical care of the deteriorating patient.

Commit to a culture of life-long learning, maintaining a professional portfolio of evidence relating to scope of practice, service specific capabilities and competencies, relevant to the role’s clinical setting, in line with National Outreach Forum (NOrF) Critical Care Outreach curriculum frameworks or credentials (including with HEE or equivalent) to be evidenced during personal development plan / appraisal.

Leadership

Promote team working, build rapport and collaborative working practices within the multidisciplinary team. Liaise with inter-hospital departments and personnel across organisational and professional boundaries ensuring effective communication with other disciplines and organisations.

Provide highly visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice.

Act with integrity applying sound personal judgement in all interactions. Encouraging teamwork, co- operation, supporting both junior colleagues and or peers in their growth and development.

Recognise and initiate appropriate action when own and others behaviours / practices is not acceptable and or in line with organisational CARE values. Deliver feedback that is positive and constructive, approaching unacceptable behaviours, errors / mistakes as opportunities to learn and develop from.

Act as a role model demonstrating high standards of holistic care. Actively support the development of clinical leadership across the Trust.

Actively participate and or facilitates the clinical supervision and wellbeing support of peers, trainee ACP/ANPs, and other members of the MDT or students as required within services.

Support team members from a managerial perspective including direct line management responsibilities (where required), such as supporting wellbeing, team member professional development, appraisals and conduct or performance management, or absence management and support (department specific).

Education

Develop a culture of organisational learning to inspire future and existing staff. Act as a role model, educator, supervisor, coach and or mentor, seeking to instil and develop the confidence in others.

Actively participate in departmental & Advanced Clinical Practice Grand Round to promote multi- professional and disciplinary learning.

Supporting the wider team to build capacity and capability through work-based and inter-professional learning, and the application of learning to practice.

Critically assess and address own CPD learning needs as an established ANP/ACP, working to a personal development plan that reflects the breadth of professional development across the four pillars of advanced clinical practice, relevant to own clinical service.

Delivers education for trainees, peers, external courses, conferences, or via HEIs, within own scope of capability and competence; utilising teaching skills to facilitate education and learning through a variety of methods (face to face /virtual / lectures / simulations / publications etc) and seeking to instil confidence in others and share best practices.

Engage with, appraise and respond to individuals’ motivation, development stage and capacity, working collaboratively to support health literary and empower individuals to participate in decisions about their care and to maximise their health and well-being.

Research

Develop a “knowledge rich enquire culture” where research values are promoted and encouraged.

Identify gaps in own knowledge or evidence bases, explores how these may be addressed via safe and pragmatic solutions, seeking to understand needs for further research to strengthen evidence.

Evaluate and audit own clinical practice, selecting and applying valid, reliable methods, then sharing and acting on the findings as appropriate with support as required, measuring value and impact of their Advanced Nurse (Clinical) Practice role.

Critically appraise and synthesise the outcome of new and or relevant research, using the results to underpin own practice and to inform that of others.

Take a critical approach to identify gaps in the evidence base and its application to practice, alerting appropriate individuals and the organisation to these and how they might be addressed in a safe and pragmatic way.

Actively engage in research activities, upholding good research practice guidelines, in order to develop evidence-based strategies that can be applied to enhance quality, safety, productivity, value or wellbeing. This includes being involved as an educator, recruiter, investigator, and/or developing bids to apply for research funding.
