Assistant Patient Pathway Coordinator Diabetes and Endocrinology- Band 3 (Fixed term/ Secondment - 10 months)

An exciting opportunity has arisen for a motivated individual to join our Diabetes and Endocrinology Team to assist the Patient Pathway Manager and Patient Pathway Coordinators by carrying out delegated responsibilities for a range of operational activities, working autonomously using initiative and their specialist knowledge and experience of the service to make decision.

Main duties of the job

To specialise in specific modality appointment bookings but have an overview and knowledge of all booking processes.

To ensure full compliance with scheduling guidelines, protocols and clinical targets whilst identifying and resolving breach situations and the consequences of changes to timetables and potential impact on capacity.

Detailed job description and main responsibilities

To be the point of contact for all administrative issues relating to patients’ pathway of care.

To work as part of a multidisciplinary team and provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

To ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.

To provide a consistent approach across the Trust; Standard Operating Procedures (SOPs) will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered.

Qualifications

Essential criteria
  • GCSE or equivalent
  • Educated to A-level, or NVQ Level 4 in Business Administration/Customer Care or acquired equivalent experience

Experience

  • Experience of using of IT systems and patient data systems
  • Experience of scheduling outpatient appointments and/or admissions
  • Experience of working with patients and providing information regarding their appointments or treatment team
Desirable criteria
  • Experience of working in healthcare setting
