Diabetes Specialist Nurse

The post-holder works as an expert practitioner and is responsible for the delivery of patient-focused programmes of care, which address the complex and changing health and well-being needs of the client group in his/her clinical area.

The post-holder displays excellent communication and leadership skills and is responsible for the day-to-day line management of the team for which he/she is responsible. 

Provide advice and support to junior staff in all aspects of care delivery and maintain high standards of care in diabetes nursing.

Provide evidence base education for patients and significant others and promote self-management at home and during their hospital stay.

Demonstrate specialist expertise and professional knowledge acquired through post registration academic, clinical, leadership and management development. Works as a role model for professional practice.

Work with lead nurse and matron, utilising the umbrella of Clinical Governance to maintain/improve and develop high standards of patient care. Place the patient’s experience at the core of service delivery. Monitor and audit the quality of care through a range of strategies, including risk management, incident reporting, complaints, research and effectiveness monitoring, and clinical audit.

Works collaboratively with the multi-disciplinary team to develop and maintain a standard of care in relation to teenage and young adult patient group.

Works both as independent practitioner and as an effective member of the multidisciplinary team.

Leads maintenance of patient advise line in line with agreed service provision.

  • Registered Nurse (Pt 1/12) ((RSCN (Pt 8) if appropriate))
  • ENB 998/Slice, Diabetes course, or equivalent

  • Advance history taking and non-medical prescribing course

Experience

Meini prawf hanfodol
  • 5 years post-qualification, of which 2 years must be in speciality - and the use of diabetes technology
  • Experience in facilitation, teaching, assessing and coaching
  • Experience in insulin pump therapy
  • Experience in managing team, workload and budget
