Job overview

We are looking to appoint one new Consultant Geriatrician to join our dynamic and enthusiastic team.  This job is a split rotational post predominantly involving work with the Urgent Community Response Service (hospital at home virtual ward and into care homes) and with some input into the acute elderly medicine department at the Royal Berkshire Hospital in Reading. This is a 10 PA consultant post in Elderly Care Medicine. As a forward thinking and adaptable department, we would welcome applications from candidates wishing to work less than full time. The post will include participation in the Elderly Care Physician on call rota at approximately 1:16.

The post holder will work alongside another both in hospital teams while covering an acute medical ward and the multidisciplinary UCR team while covering the virtual ward.  This job allows the applicant to work alongside all our consultants in both RBH and BHFT and alongside both inpatient and outpatient multidisciplinary teams.

 

The simulation date is the 6th September 2024 and the Interview date is 18th September 2024.

Main duties of the job

We post holder will be expected to:

 

To work with our Urgent Community Response Virtual Ward supporting the daily huddles, weekly ward rounds and reviewing patients in their own homes when needed.

Support and develop the UCR team; by supporting training of nurses & therapists as well as providing clinical supervision.

Provide, with colleagues, Comprehensive Geriatric Assessment service for frail older patients admitted under the care of the Elderly Care team.

Manage the patients on an acute Elderly Care ward, working closely with colleagues in the multidisciplinary team and providing consultant support to the medical team.

Provide on-call cover for the Elderly Care Physician (ECPOD) on call on a 1 in 16 rota. On-call is Category A with 3% on-call intensity supplement. Specific fixed weekday to be confirmed.

Prospective cover for each other’s annual leave will be shared equally between the consultant team.

Active participation in monthly clinical governance meetings within the department.

Participation and involvement in meetings and other business in the elderly care service.

Responsibility for the educational supervision of one trainee and clinical supervision of one trainee.

Development of sub-specialty interests in community geriatrics with a focus on improving services within the virtual ward

Education and Qualifications

Essential criteria
  • Full registration with the GMC/eligible for registration Within 6 months of CCST in (Speciality) at interview date
  • Membership of the Royal College of Geriatric Medicine of the United Kingdom or equivalent
Desirable criteria
  • Higher degree e.g. PhD/ MD submitted/awarded

Clinical Experience, Knowledge & Skills

Essential criteria
  • Fully trained in Geriatric Medicine and General Internal Medicine
  • Previous responsibility for clinical governance and GMC Good Medical Practice
  • Sub-specialty experience in Community Geriatric Medicine
  • Ability to make appropriate assessments in the management of complex medical problems in older people and coordinate the completion of a comprehensive geriatric assessment
  • Ability to make decisions at consultant level
Desirable criteria
  • Knowledge/experience of Care Home medicine services

Audit Management & IT

Essential criteria
  • Ability to work within clinical governance guidelines
  • Undertake audits and present data as required
  • Good IT skills, use of patient and hospital database
Desirable criteria
  • Evidence of clinical leadership role demonstrating accountability for quality of care, financial controls and efficient management of workforce

Research, Teaching Skill & Experience

Essential criteria
  • Evidence or providing good teaching and supervision to trainees
