Patient Pathway Coordinator (CAT9, Diabetes & Endocrinology)
We require a passionate, enthusiastic, reliable and hardworking individual to join our team as a Patient Pathway Coordinator to join our team.
We are looking for an excellent and proactive team player who can offer commitment in providing support to clinical and non-clinical staff.
The ideal candidate will have strong customer service skills and be willing to multi-task under challenging conditions and have the ability to self-organize and assist with work from those within their team.
Main duties of the job
You will be the point of contact for all administrative issues relating to patients' pathway of care, working as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
You will provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

Detailed job description and main responsibilities
The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will supervise and allocate work to the PPC team.
The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact. To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

Education/Qualifications
Essential criteria
- Educated to A-level, or NVQ Level 4 in Business Administration/Customer Care or acquired equivalent experience
- GCSE or equivalent in English and Maths, grades A-C
Previous Experience
Essential criteria
- Experience of using full range of IT systems and patient data systems
- Experience of scheduling outpatient appointments and/or admissions
- Experience of working in a multidisciplinary team
- Experience of working in a healthcare setting
Desirable criteria
- Experience of supervising staff
Applicant requirements
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
