Job overview
We are seeking to recruit an experienced mental health practitioner to be an integral part of the Mental Health Integrated Community Service (MHICS) for Slough. The postholder will be responsible for working closely with General Practitioners and the primary care team to provide biopsychosocial assessments, brief interventions and care navigation for people with significant mental health issues living in the locality.
This is an exciting opportunity to be involved in the development and shaping of a major new and innovative approach to providing mental health care within the community, working closely with all relevant providers within the individual’s local community network.
Frimley Integrated Care System has been awarded new NHS funding to transform community mental health services for people with significant mental health problems. A ground-breaking new model is being implemented within Berkshire Healthcare NHS Foundation Trust and Surrey and Borders Partnership NHS Foundation Trust to benefit those patients within the primary care setting.
The funding has enabled new Primary Care Network (PCN) mental health teams to be developed. Each PCN, comprising a population of approximately 30 – 50,000 patients, hosts a multi-disciplinary team which will work in close collaboration with a wide range of stakeholders to provide support for people with significant mental health difficulties to meet their needs, closer to their communities and homes.
Main duties of the job

Working as a senior level practitioner, supervising junior staff members and coordinating the day-to-day functioning and delivery of the clinical service.

Working with people with significant mental health problems, whose mental health needs can be best met within primary care and whose difficulties are best understood within a biopsychosocial model.

Building and facilitating closer links between primary care networks and Community Mental Health Teams, Social Care teams, community services and a wide range of voluntary sector organisations, to achieve seamless transitions and support for people with significant mental health needs.

The “must haves” for you to be considered for this role:
- Qualified as a registered Mental Health Nurse, Social Worker, Occupational Therapist or High-Intensity CBT Therapist.
- Experience of supporting people with significant mental health problems in community settings.
- Post registration training and development in relevant psychosocial interventions.
- Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes.

Education/Qualifications
Essential criteria
- Current professional registration: Registered Mental Health Nurse, Social Worker, Occupational Therapist or equivalent
- Evidence of relevant post registration training and development in particular motivational interviewing, brief solution focussed interventions, cognitive and behavioural approaches and psychosocial interventions
Desirable criteria
- Independent and supplementary Prescriber
- Recognised Post Graduate clinical or leadership qualification relevant to the role
- Mentor and mentorship training
Experience
Essential criteria
- Minimum 5 years post registration experience in working with people with mental health needs.
- Community experience in mental health services for adults and older age adults with severe / significant mental illness
- Experience of providing a range of evidence based interventions to people with a variety of mental health problems and serious mental illness
- Considerable experience in the assessment and management of clinical risk.
- Experience in physical health assessment for people with mental health needs.
- Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes.
- Evidence of Interdisciplinary / multi-agency working experience including primary care and community/ voluntary sector.
- Experience of leadership and development of junior colleagues.
Desirable criteria
- Proficient in clinical audits and research.
- Experience of working in primary and secondary mental health services.
- Experience in the planning and delivery of training and practice developments to staff.
- Experience in service evaluation and improvements.
- Experience working in a CAMHS and/or OPMH service
- Direct Management/Supervisor experience.

- Advanced understanding of the mental health needs of adults.

- Specialist knowledge of psycho-social interventions and their appropriate use.
- Working knowledge of appropriate assessment tools.
- Working knowledge of the policies and guidelines pertinent to the planning and delivery of community MH care to adults with mental health needs.
- Recovery focused practitioner able to apply recovery principles to adults and older people, understanding of ‘strengths- based’ approaches
- Clear understanding of psychotropic medication and the monitoring of effects and side effects.
Desirable criteria
- Knowledge of practical application and use psychological therapies
- Awareness of the impact of the health economies on service planning and delivery.
- Specialist knowledge relating to MH needs of young adults age 18-25 and to the needs of those diagnosed with, or with presenting traits of, personality disorder
