 We have exciting opportunity for you to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust, the largest community trust in the country.

Part of a wider Early Intervention programme, which has delivered in the last 12 months: 77000 fewer acute bed days used, 19000 fewer non-acute bed days and people spend 11.5 fewer days in the system. Overall having an impact of £25.8million saved for Birmingham to reinvest in Health and Social Care developments.

Six health and social care organisations joined forces in the region to deliver this truly integrated service. Alongside BCHC, other partners include Birmingham City Council, University Hospitals Birmingham, Birmingham & Solihull Mental Health Foundation Trust, Birmingham and Solihull CCG and Sandwell and West Birmingham CCG

 

 

Main duties of the job

The post holder will work with patients as part of a multidisciplinary team to facilitate their rehabilitation and enable them to achieve the optimum possible independence, within the parameters of the Early Intervention Community Team (EICT)

The post holder will support the clinicians in delivering the rehabilitation programme following initial assessment, in the patient’s own home or in a community facility.

The post holder will be expected to work shifts as required over 7 days per week in line with service delivery.

Qualifications/Training

Essential criteria
  • Education to GCSE level in 5 subjects to include English Language or equivalent
  • Evidence of fluency in English both written and verbal.
  • NVQ Level 3 in care or equivalent.
  • Evidence of continued learning and development.

Experience

Essential criteria
  • Caring for the older/disabled adults in the community or in-patient setting.
  • Previous Experience of working with adults.

Skills/Knowledge

Essential criteria
  • Skills for communication on complex information requiring developed interpersonal and oral/written communication skills
  • Theoretical and practical knowledge of a range of therapeutic procedures and practices
  • Willing and able to undertake training appropriate to the role
  • Ability to undertake a range of duties and to learn new tasks including clinical procedures where relevant.
  • Knowledge of Health and Safety issues in the workplace
