Capacity Manager - S2R- Intensive Community Rehabilitation Team.
We are looking to recruit a highly skilled clinician who will support the S2R inpatient teams and Intensive Community Rehabilitation team, with supporting care and maximising patient flow through out of area placements through to inpatient rehabilitation units. Capacity manager will work alongside discharge coordinators, managing effective patient flow by identifying capacity and creating healthy movement.
The capacity manager will have a day-to-day operational role, ensuring safe facilitation of patient flow. The successful candidate will work with our local teams and private partners to ensure service users who access rehabilitation are supported with a robust and supportive discharge plan. The candidate will also work with the inpatient teams to support sourcing community rehabilitation to reduce length of stays.

They must be able to work independently, be able to undertake comprehensive assessments and robust discharge plans, working collaboratively with service users, carers and clinical teams. Possession of comprehensive IT skills are essential as there will be a requirement to provide comprehensive performance data regarding clinical activity.
The successful candidate will be expected to work independently, and private partners to assist in the implementation of robust and supportive discharge pathways. This could mean completing referrals, assessments, presenting cases for funding to panels, attending ward reviews and bed meetings, to assist in improving the bed flow.
