Job overview
To play a key role in the development, management and co-ordination of the Multi-Disciplinary National Hepatitis C Service (HCV MDT), ensuring the delivery of coordinated and timely care for children and families referred to the multi-disciplinary team and coordination of the Hep C Joint Paediatric Tracking project.
To ensure the HCV MDT meets the requirements set out by NHS England relating to data collection, audit and submission of data
The postholder will support the development of pathways and protocols, coordinating meetings, facilitating aspects of all the ODN projects and monitoring project activities to ensure progress is made and reported on.
The postholder will liaise with appropriate personnel to ensure that individuals and families are followed up and records of action taken and required are maintained.
To coordinate the joint paediatric HCV tracking proiect, following up children and young people to determine HCV status. Supporting practitioners, providing HCV information regarding testing, treatment, referral pathways to ensure any support or treatment is followed up on, as necessary.
The postholder will work remotely, with access to a hot desk and office space if required
Main duties of the job
· Facilitate the development and implementation of shared working agreements between organisations involved in the project and establish appropriate governance with the development of memorandum of understanding, as required.
· Develop and maintain an accurate system of recording case details, actions taken, referral information and outcomes that can be used to regularly report on project progress and results.
· Organise and manage the project, ensuring the necessary communication mechanisms, support staff, services, facilities and data management systems are in place.
· The post holder will adhere to the Data Protection Act and maintain patient confidentiality at all times seeking information and support from colleagues as required as the project develops

Qualifications
Essential criteria
- Minimum 2 ‘A’ Levels
Desirable criteria
- University degree 2.I in a relevant subject or equivalent experience in a health-related subject
- Relevant project management qualification
Knowledge
Essential criteria
- Knowledge of WHO’s elimination target and NHSE elimination plan for Hepatitis C
- Significant experience of inter-agency, partnership working and developing relationships
- Experience of service development in a health-related field
- Understanding of health protection, the inclusion health agenda and access to healthcare
Desirable criteria
- Up to date knowledge and experience of working with partner agencies across the system; including NHS, local authority, community and voluntary sector
- Experience of working as part of an Operational Delivery Network
