Regional Transition Pathway Development Nurse

Job overview

An exciting opportunity has arisen for a dedicated transition improvement nurse to join a busy and dynamic specialist nursing team within the Midlands regional haemoglobinopathy service. The post will be based onsite at both Birmingham Children’s Hospital and City Hospital Birmingham. The post holder will be required to manage their time between both locations to allow collaborative working to improve transition pathways within the Midlands network. The role will also require some travel to centres across the Midlands.

Main duties of the job

 Together the team will be responsible for the early recognition and management of the healthcare transition of children and young people living with red cell disorders and in the delivery of transition quality goals. The post holder will work collaboratively to review the existing regional service and develop and implement a robust transition policy for the region. To navigate pathways for the region.
The post holder will be a champion for smooth transition, listening to and advocating for patient and family experiences of transition across the region. They will offer support and advise at MDTs (Multi-Disciplinary Team meetings) to help develop the regional services. The role will act as a professional advisor in the on-going training and development of team members within the region, working in partnership with the regional education lead nurses to implement and embed the transition education package.
The post holder will be professionally accountable to the lead Nurse for Haem/Onc. The post holder must work collaboratively with the transition leads at BWCH and SWBH.
This role is a patient facing clinical role, with the requirement for the applicant to be a registered healthcare professional.

Qualifications

Essential criteria
  • Registered nurse - with evidence of providing developmentally appropriate healthcare for adolescents during the transitions process
  • Evidence of post graduate diploma in relevant area and evidence of specialist qualification to masters level or equivalent experience
  • Evidence of ongoing, dynamic continuing professional development within the specialty, linked to demonstrable clinical competencies
Desirable criteria
  • Assessing and mentoring qualification

Knowledge & nature of experience

Essential criteria
  • Expert experience at a senior nursing level within HBO or transition services, including within the clinical environment
  • Previous experience of change management and problem solving
  • Experience of handling clinical incidents and complaints
  • Demonstrates Compassion in Practice
  • Understanding of professional and current issues in children’s nursing and healthcare, e.g. Laming Report, Francis report, CNO’s vision for Compassion In practice
  • Safeguarding procedures
  • Evidence of fulfilling CPD (Continuing Professional Development)
  • Experience of working as a CNS/Improvement Manager
Desirable criteria
  • Knowledge of the key principles underlying functional clinical networks and understanding of service specification for SHT’s.
  • Experience of leading and motivating
  • Expert knowledge relevant to the speciality/field or practice

PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE

Essential criteria
  • Good computer skills using Microsoft Office (Word, Excel, Outlook and Power Point) and IT systems such as Vesper, ESR, Ormis, Lorenzo, E-Roster, Oracle etc.
