Job overview

The post holder will have a key role in maintaining an effective service to support clinical and corporate services in the effective and timely notification, management, investigation, and response to all Patient Safety Incident’s.

 

The postholder will work closely with staff and managers across Trust services to ensure all pertinent details about each incident are accurately recorded in the Trust’s incident management system (Datix). They will ensure timely reporting of all Patient Safety Incident Investigations to the STEIS electronic reporting system. They will work with investigation leads to provide facilitated support and ensure the timely provision of all incident investigation updates, investigation reports, action plans and quality assurance documents.

Main duties of the job

To support the Patient Safety Team in the daily management of the incident reporting process and associated work streams across the Trust

 

To act as the day-to-day point of contact for staff regarding the reporting for all incidents, dealing with queries and providing advice and support as required

 

To carry out a daily review of all incidents reported on Datix to challenge the completeness of information, accuracy or severity, and to ensure the appropriate action and level of response has been initiated 

 

Effectively deal with all written and verbal enquiries responsively, deciding upon and initiating appropriate follow up action required promptly. 

 

To escalate any identified potential PSII’s to the Patient Safety Manager and Deputy Patient Safety Manager. 

To report all agreed PSII's to the STEIS system

To identify incidents requiring discussion at the Weekly Meeting of Harm & Learning for potential PSIRF response

 Ensure the Datix system is continually updated to provide a ‘live’ record of progress and accurate reporting

Identify non-compliant areas for escalation to managers and leads, where incident responses have not been completed within set timescale or there are outstanding actions after the due completion date

To contribute to the effective integration of patient safety/clinical risk with other quality governance and accountability programmes and functions

Support the process for reporting to Learning from Patient Safety Events (LFPSE)

Qualifications & Education

Essential criteria
  • Educated to degree level or equivalent experience.

Knowledge and Training

Essential criteria
  • To have IT and high-level administration skills being able to analyse data, contribute to reports and be proficient in the use of databases, Outlook, Word, and Excel (Microsoft office).
Desirable criteria
  • Knowledge of NHS Governance process, in particular incident management

Experience

Essential criteria
  • Understanding of the NHS governance and quality agenda
  • Ability to work with colleagues at all levels and to liaise on sensitive and important issues
  • Experience of managing complex and sensitive information
Desirable criteria
  • Knowledge of the DATIX electronic incident reporting system
