Community Nurse Frailty and Dementia– Learning Disabilities – Band 6
An exciting opportunity has arisen for experienced and highly motivated individual to join community city wide service within the Learning Disabilities Service.
Due to the nature of the role this is open to Registered Nurses Learning Disability who have experience in supporting service users with frailty and or dementia
The role involves providing specialist support and guidance to adults with learning disabilities who are frail and/or diagnosed with dementia and supporting their carers. The successful candidate will provide clinical services through the effective management of an active caseload.
You will be an experienced and flexible individual with excellent communication skills and a passion for multi disciplinary team working.

To work within the framework of a multi-disciplinary team to provide support and guidance to adults with learning disabilities and their carers.
To work within the framework of health facilitation (level one and two).
To co-ordinate a clinical service under the supervision of Senior Community Nurses.
To provide support to adults with learning disability who have dementia and/or frailty.

Main Duties
1. To manage and co-ordinate a caseload under the supervision of the Senior
Community Nurse.
2. To focus on individual health outcomes for people with learning disabilities and offer
support to access health services.
3. To promote service development and planning within mainstream services supporting
them to achieve Valuing People targets and examine initiatives to reduce health
inequalities.
4. To assess, plan, implement, evaluate and review programmes of care and ensure
measurable outcomes and discharges are efficiently and effectively carried out.
To have a working knowledge of the Mental Health Act, Valuing People and
Human Rights Act and a general awareness of other legislation that impacts on
the lives of people with a Learning Disability including older adults legislation.

To assist individuals with learning disabilities and their carers, giving practical help and
advice on such matters as:

• Diet
• Promotion of Continence

• Respite Care Health
education
• Practical Problems e.g., lifting
Mental Health needs Physical
Disabilities
• Sensory Disabilities

• Relationship and sexuality
Parenting
• Care Programme Approach Co-Ordination
To act, here necessary, as an advocate for the person with a learning disability,
bearing in mind the rights, needs and dignity of the individual and development of
self esteem.

8. To participate in meetings, case conferences and clinics, where appropriate.
To liaise with all professionals, statutory and voluntary agencies working with people
with learning disabilities e.g., Primary Health Care Teams, Social Care & Health,
Education Services and others as required.

To liaise with all appropriate professionals, carers and Service User with regard to
pre and post admission support to respite care/assessment/treatment/
Acute/Hospitals.

11. To refer clients to other agencies as appropriate.
12. To work within the principles of clinical governance.
To work flexibly in response to Service Users' individual needs, i.e., evenings,
weekends and public holidays.

14. To promote the values and principles of the organisation in relation to diversity.
15. To be sensitive and proactive in meeting the needs of the service user from a variety
of ethnic cultures

17. To participate on the On-Call Out of hours Duty on a rotational basis

- Post qualification training
