We are looking for enthusiastic and dynamic individuals who are able to work within a multi- disciplinary team, working alongside GP’s and other primary care professionals who strive to achieve  the best outcomes for individuals within the local community.

You will be responsible for providing mental health triage, assessment and outcomes based intervention to meet service users needs within the Primary Care Network. You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
